'This is a war': NYC doctors describe fight against coronavirus as cases surge
“Our hospital has never, ever, ever seen anything like this,” one doctor said.

Overfilled waiting rooms packed with people who are contagious. Patients waiting six hours to be seen. Others on stretchers waiting 50 to 60 hours for a bed. Doctors desperately trying to get more ventilators. That is what it’s like to be on the front lines of the coronavirus pandemic at a public hospital in New York City, Dr. Rikki Lane, an emergency room doctor at the Elmhurst Hospital Center in Queens, said.

Our hospital has never, ever, ever seen anything like this,” said Lane, who has worked for more than 20 years at Elmhurst, a public hospital with 545 beds.

Lane said the emergency department has been “overwhelmed” for about three weeks and the hospital is in desperate need of help as the coronavirus spreads across the city, which has become a fast-growing epicenter of the virus with more than 21,000 known cases and 281 deaths as of Thursday.

The hospital has spent weeks now expanding the areas within the facility that it has been using to house the coronavirus patients, but the ever rising rush of patients has been “inconceivable,” she said.

Whatever space we create is immediately filled and overfilled,” she said.

Doctors at New York City hospitals working in the fight against the coronavirus described a desperate battle to care for patients as cases continue to surge, with the peak of the disease still weeks away.

The hospital needs critical help with “everything along the supply chain,” including supplies, staff, ventilators and bed space, Lane said.

The need grows exponentially every single day,” she said.

In the worst day that Elmhurst has seen so far, the virus claimed 13 lives in one day, which Lane said was "unprecedented."

"All of this is unheard of," she said.

The doctor said she has feared New York City could become as overwhelmed as Italy, where the virus quickly spread to more than 75,000 known cases and more people have died than in China.

I know actual true horrors are coming,” she said.

“It’s just like a tidal wave,” another Elmhurst Hospital doctor, who spoke on condition of anonymity, told THE CITY, an independent, nonprofit news organization. “The only beds we’ve been able to free up are people who have died.”

Gov. Andrew Cuomo has said New York may need up to 140,000 hospital beds as the virus reaches its peak and has urged the federal government to send supplies, including tens of thousands of ventilators, to the state.

This week, hundreds of thousands of supplies were distributed across the state for immediate needs, including a million masks to New York City, but that will only last so long, Cuomo said.

The federal government was sending 2,000 ventilators to New York City, but the city is asking for 15,000, Mayor Bill de Blasio has said.

In Brooklyn, a doctor who works at a large hospital system said there had been critical shortages of swabs and masks.

The doctor, who asked to remain anonymous out of fear of retaliation, said hospital workers themselves are at high risk and she was scared of getting sick and infecting her family.

"I am scared to hug my own children," she said.

"We need space and beds. This is a war. I'm going into war and trying not to get killed," she said. "We are trying to go to work and not to die."

The city is also adding a new makeshift morgue ahead of the virus’ surge.

“I know the morgues push a really strong emotional button, obviously. We’re all humans,” de Blasio said. “It’s a very troubling thing to see, and it makes it very immediate, very visceral. It’s going to be very, very painful.”

Anthony Almojera, the vice president of the New York City Fire Department's EMS officers union, said EMS services had been receiving record calls.

"Last night we saw over 6,500 calls, the night before 6,400," he said. "The last week and a half we’ve been at 5,000 or more every day. That's more than 9/11. In the last two days, we’ve set records."

On Wednesday, Elmhurst hospital received about two dozen doctors and nurses from other hospitals, and a number of ventilators that help critically ill patients with their breathing, City Councilmember Francisco Moya (D-Queens), told THE CITY.

It’s very accurate to say it’s overrun. I’ve spoken to workers at the hospital and you can hear it in their voice — it’s a sign of desperation that they need help,” said Moya, who was born at the hospital, used to work there as business development director and now represents the district it’s in.

Christopher Miller, a NYC Health + Hospitals, spokesperson told THE CITY: “Elmhurst is at the center of this crisis, and it’s the number one priority of our public hospital system right now.”

NYC Health + Hospitals, the city’s public hospital system, said in a statement to NBC News it was “working day and night to ensure that all our patients receive the care they need.”

There were more than 1,000 ventilators across the system and NYC Health + Hospitals was receiving an additional 400 from the city’s office of Emergency Management, it said.

The hospital system said it had "adequate supplies at the moment but are fully cognizant of the nationwide scarcity of resources" and "reports of lack of personal protective equipment and ventilators in our system are false."

“The public health system is working with all local, state and federal agencies to ensure that resources are strategically allocated throughout to accommodate the surge caused by COVID-19,” the hospital system said. “We are committed to our mission to care for all New Yorkers regardless of immigration status and ability to pay, and are focused on keeping all our patients and staff safe.

Global cases surpass 400,000

Figures collected by Johns Hopkins University suggest 407,485 have been infected and, since the data is based partially on official releases, the true extent could be even greater. The institution says 18,227 people have died, while 104,234 have recovered.

Yesterday 3/23/20:

Here is a reminder of the incredible rate at which the spread of the coronavirus is accelerating.

Earlier, the World Health Organization explained it in these simple terms:

  • It tool 67 days from the first reported case or Covid-19 until the first 100,000 cases were confirmed
  • It took 11 days for until the second 100,000 cases were confirmed
  • It took just four days for until the third 100,000 cases were confirmed

===

So now it's one day to advance 100k?

RENO, Nev. (KOLO) -- Triad Technologies employees are working non-stop to produce portable ventilators.

A ventilator produced at Triad Technologies.

"All we're doing is producing as fast as we can, trying to get these things out to people that need them," said Triad Technologies C.E.O., Greg Latimer.

He says a Vortran Medical Technology manager placed an order for 10,000 on March 10, 2020.

"While we were running the 10,000 he said, 'I'm going to send another order over for 30,000 right after this and beyond that we're not quite sure we're planning for a run of 50,000 as the demand just continues to go up," Latimer said about his conversation about the orders.

Latimer said of the ventilators he's making, "This thing will do everything that a $12,000 respirator ventilator will do. However, it doesn't monitor the respiratory function like one of the $12,000 respirators although this one is $100."

Triad Technologies will begin production of what's called Nestable Tip Racks. Scientists will use them to search for a COVID-19 vaccine.

Latimer says all of the profit he makes including from ventilator production is donated to charities including the Children's Cancer Foundation and the Crisis Pregnancy Center.

"This is the horror story we absolutely did not want to have happen," Justice said during a news conference Thursday.
Justice said he has appointed Dr. Clay Marsh to coordinate the state's response and preparedness as the virus continues to spread.
"It gives us one more layer of expertise and one more layer of affiliation with hospital," Justice said.
By the numbers: CNN has counted at least 52 Covid-19 cases in West Virginia as of Thursday afternoon.

I had to run down To McKenzie Bridge this morning, and 126 was noticeably thinner of traffic than usual. I did notice that the few trucks on the road were running in pairs, which I found very interesting. There were only two cars at the McKenzie River Trailhead, which is way down from a few days ago.

Altogether, there were only a dozen or so cars on the road, and one RV heading west.

Coronavirus Pandemic Update 44: Loss of Smell & Conjunctivitis in COVID-19, Is Fever Helpful?

•Mar 26, 2020 Dr. Roger Seheult

Coronavirus (COVID-19) Update 44 with Roger Seheult, MD of https://www.MedCram.com There is new evidence that loss of smell (anosmia) may be an indicator of COVD-19 infection. New reports also suggest that COVID-19 may cause conjunctivitis (pink eye), and be potentially spread through contact with the conjunctiva. Dr. Seheult discusses evidence about how untreated fever may be beneficial for fighting viral illnesses such as coronavirus. Italy’s northern Piedmont region said on Thursday that 50 people had died there from coronavirus in the last 24 hours, numbers which were omitted from the national tally released by the Civil Protection Agency because they arrived too late.

The omission means that daily deaths amounted to 712 on Thursday rather than the 662 officially reported, and marked an increase from the 683 registered the previous day.

Total deaths since the start of the outbreak are therefore 8,215, rather than the 8,165 reported.

Coronavirus outbreak: New York confirms 6,400 new COVID-19 cases | FULL

•Streamed live 4 hours 3/26/2020 ago Gov Cuomo

Global News

New York Governor Andrew Cuomo told reporters at a news conference on Thursday that almost "any realistic scenario" in the COVID-19 pandemic will still overwhelm the state's existing hospital capacity, and the goal was to add hospital beds wherever it could. Cuomo also outlined the latest number of COVID-19 cases, saying that there are 6400 new cases and that the "curve continues to rise," and the spread continues across the state. He also noted the state's death toll stood at 385, up from 285 a day earlier. Cuomo said he was "disappointed" with the stimulus bill passed by the U.S. Senate Wednesday night, and called the actions of the Senate "irresponsible" and "reckless."

Rhode Island is requiring anyone traveling from New York to self-quarantine

Rhode Island Gov. Gina Raimondo today said she is “mandating, not suggesting” a 14-day self-quarantine for anyone traveling to her state from New York.

The executive order – effective immediately – also applies to anyone who has traveled to New York in the last 14 days.

The governor said she plans to execute the mandate by stationing members of the National Guard at bus terminals and train stations and will require anyone arriving from New York to check in and provide their contact information.

State police have also been instructed to flag down cars with New York state license plates as they come into Rhode Island.
“This is unusual… this is radical,” the governor said of the plan to execute the mandate.
Raimondo said Rhode Island has added 33 cases of coronavirus since yesterday, bringing the state’s total to 165 cases.

One of the girls in the Cape Cod COVID group said she called her mortgage company to ask about delaying her mortgage payment. They said fine she could do it for three months...but at the end of the three months she had to pay it back in full....what a bunch of ass wipes! How are people supposed to do That?

With 10 year treasuries very low, this could be a blessing in disguise. Check into a refinance and possibly save some money per month even if you factor in the late charges. It is worth a little bit of your time. Best

Canada says it is strongly opposed to the US putting troops on the border

Canadian Prime Minister Justin Trudeau confirmed that the US is requesting American troops be placed within 20 miles of the US-Canada border to help with border surveillance especially at irregular crossings. Trudeau indicated his government did not believe that was necessary at this time, but that talks were ongoing.
“Canada and the United States have the longest un-militarized border in the world and it is very much in both of our interests for it to remain that way. We have been in discussions with the United States on this,” Trudeau said during a news conference outside his Ottawa home where he remains in quarantine.
Later at a government news conference in Ottawa, Canada’s Deputy Prime Minister Chrystia Freeland revealed Canada had made its opposition known to many levels of the US government, including the Defense Department and US Homeland Security.
"Canada is strongly opposed to this US proposal and we've made that opposition very, very clear to our American counterparts,” she said. “We understand the concerns of our American neighbors and friends. What we have said is we really do not believe at all that there would be a public health justification for you to take this action.”
In blunt terms Freeland underscored that, in her opinion, this was not the way treat a trusted friend and military ally.
Some context: Canada borders 13 American states and before the Covid-19 crisis the two countries shared one of the most lucrative trading relationships in the world. Both countries jointly decided to close the border to nonessential traffic earlier this month. Essential commercial trade and traffic has continued with enhanced medical screening.

1. Anyone else hear on Fox News that DC doctors want auto DNR on any intubated patient? Saying not worth risk to staff esp. due to lack of PPE

2. So although I am working remotely - thank you Lord - the factory is still running as an "essential business" due to industries supported - medical, telecom, infrastructure. There are so many "essential businesses" and businesses that support essential businesses that this shutdown is, at most, only 50% effective. Companies are scrambling to figure out how to be considered essential so as to stay open. Greed!

Well wouldn't you know now three poor souls at the plant have been diagnosed. Each one takes another 12-15 employees into quarantine with them due to having been in contact (they're supposed to stay 6' away but I don't know how)

Pretty soon there won't be anyone to run the machines... And the virus is still spreading.

Meanwhile day cares, those festering germ pits, stay open because mom and dad still have to go to work. Ay dios mio!

Please stay off the Appalachian Trail

The Appalachian Trail, given its ever-increasing popularity over the past weeks, is no longer a viable space to practice social distancing.
In these unprecedented times, I am making an unprecedented request: please stay away from the Appalachian Trail (A.T.). Whether your hike is for a couple of hours or a couple of days. Staying away from the Trail minimizes the spread or contraction of COVID-19.

In a time when social distancing is necessary to minimize the spread and contraction of a dangerous virus, many have escaped to nature seeking isolation and unpopulated spaces. On the Appalachian Trail, however, what they’ve found are trailhead parking lots exceeding their maximum capacities, shelters full of overnight hikers, day hikers using picnic tables and privies, and group trips continuing as planned. Popular spots along the Trail like Blood Mountain in Georgia, the McAfee Knob area in Virginia, and Annapolis Rocks in Maryland have seen day use reach record-breaking levels. Cars line the highways leading to popular day-hiking spots on the Trail. Hiking the A.T. has become, in other words, the opposite of social distancing.

These same crowds accessing the A.T. may not know how a simple half-day hike can spread COVID-19. While hiking, they may have eaten lunch at a picnic table, taken a break in a shelter, used a privy, or shared a map or food with someone unknowingly infected with COVID-19 and carried this highly contagious virus back to their communities at the end of the day. They may not have realized that ATC staff and Trail volunteers have been recalled from the A.T. and cannot maintain the footpath, trailheads, shelters and privies that may be heavily (or permanently) impacted by increased visitor use. And, they may not be aware of the rural communities adjacent to the Trail that may not have the healthcare resources to help a sick hiker or volunteer or manage a COVID-19 outbreak should a hiker transport the virus in from the Trail.

Many day hikers see the outdoors as an escape from the stresses of these difficult times. But with crowding from day hikers reaching unmanageable levels and the lack of any staff or volunteers to manage this traffic, it is necessary that all hikers avoid accessing the Trail. The A.T. is not a separate reality from the communities in which hikers live – so, until the risk of spreading COVID-19 has reduced significantly, hiking on a heavily-trafficked trail like the A.T. potentially increases rather than reduces harm.

The ATC does not want to do too little, too late. We cannot close the Trail. We cannot physically bar access to trailheads or connecting trails. We can and do, however, urge everyone to please stay away from the Appalachian Trail until further notice.

There is an unfortunate truth about this virus: unless everyone is safe, no one is safe. So, take a walk around the block. Spend time with your loved ones. And, please, stay home.

Sincerely,
Sandra Marra

Appalachian Trail Conservancy
