Mysterious pneumonia kills 3, infects 9 in Argentina: ‘Similar to COVID’
By Adriana Diaz
September 1, 2022 2:53pm Updated

Fears of a new deadly viral outbreak have begun to spread after three people died in Argentina this week from a mysterious case of pneumonia.

Another six people have also been infected with a “pneumonia of unknown origin,” Argentine health officials reported.

Of the nine people who have been infected, eight were health workers and one was an ICU patient being treated at a private clinic in Tucumán — a small region 800 miles northwest of the capital Buenos Aires.

Health officials have expressed their mounting concerns after tests for 30 infections including COVID, flu and influenza types A and B came back negative, Tucumán health minister Luis Medina Ruiz confirmed to local news outlets Wednesday.

“What these patients have in common is the severe respiratory condition with bilateral pneumonia and compromise in [Z-ray] images very similar to COVID, but that is ruled out,” Ruiz said.

All of the infected people were employees or patients of a private clinic in Tucumán.

Meanwhile, the European Centre for Disease Control’s epidemic intelligence team has been monitoring the cases since Tuesday and scientists at the World Health Organization have also begun tracking the situation.

“It’s obviously concerning but we still need key information on transmission and hopefully [on the] underlying cause,” professor Devi Sridhar, chair of global health at Edinburgh University and author of “Preventable: How a Pandemic Changed The World & How to Stop the Next One,” told the Telegraph.

Samples of the unknown virus have reportedly been sent for testing to the Malbran Institute in Buenos Aires and Argentina’s National Administration of Health Laboratories and Institutes.

Local officials are also examining the water and air conditioning units in the area to test for possible poisoning.

The most recent victim of the virus was a 70-year-old woman who died Thursday. She had been admitted to the clinic where the infected health professionals worked ahead of a procedure.

The elderly woman has been suspected to have been “patient zero” but Ruiz clarified that those facts are still “being evaluated.”

The first patient died Monday and the second Wednesday after all suffering from “a severe respiratory condition with bilateral pneumonia … very similar to COVID.”

The first six patients started exhibiting symptoms between Aug. 18 and 22. Officials have not said when the three most recently infected people began exhibiting symptoms but noted that two are receiving clinical treatment and one is being monitored at home.

This comes nearly two and a half years after the COVID-19 outbreak began spreading across the globe. Several other illnesses have since sparked fears of another pandemic with the US recently declaring a state of emergency over monkeypox.

Health officials monitoring mysterious bilateral pneumonia cases in Argentinian province
By Theresa Braine
New York Daily News
Sep 01, 2022 at 8:43 pm

An unidentified, mystery respiratory ailment has killed three people at a private clinic about 800 miles from the Argentina’s capital of Buenos Aires.

It’s doing to the lungs what COVID does. But the total of nine patients infected have not tested positive for novel coronavirus. Or for flu, Hantavirus or more than two dozen other pathogens.

“What these patients have in common is the severe respiratory condition with bilateral pneumonia and compromise in [x-ray] images very similar to COVID, but that is ruled out,” Luis Medina Ruiz, Tucumán’s minister of health, was quoted as saying Wednesday by the Buenos Aires Times.

Those infected included eight medical staff at the private clinic in San Miguel de Tucumán. The first patient died Monday, the second Wednesday and the third, a 70-year-old woman who may or may not be “patient zero,” died on Thursday.

Health officials there are monitoring the disease and analyzing other systems such as water and air conditioning units to rule out poisoning or environmental causes, and to check for possible legionella bacteria in the air conditioning duct, the U.K.’s Telegraph reported.

The symptoms cropped up at the intensive care unit of the private hospital between Aug. 18 and 22, according to an official report cited by publication. All patients had pneumonia in both lungs.

The health center is under isolation, and no new cases have been recorded since Aug. 22, officials said.
It’s too early to raise an alarm, health officials said.

“It’s obviously concerning, but we still need key information on transmission and hopefully [on the] underlying cause,” Devi Sridhar, chair of global health at Edinburgh University told The Telegraph.

Whether it turns out to be a new infectious agent or not, the situation highlights global interconnectedness, he added.

“This shows our collective vulnerability to dangerous pathogens,” Sridhar said. “An outbreak in any part of the world — if not quickly contained — can spread rapidly given air travel and trade.”

What is the mystery pneumonia that killed three in Argentina?
Testing has ruled out coronavirus, influenza, hantavirus and about 30 other possible causes. Here's what we know.

By TZVI JOFFRE
Published: SEPTEMBER 2, 2022 11:28

Three people have died after being hospitalized with bilateral pneumonia — pneumonia that affects both lungs — in Argentina, with doctors struggling to figure out what's causing the disease, the Public Health Ministry in the province of Tucumán announced on Thursday.

The patients suffered from fever, abdominal pain, shortness of breath and muscle aches.

In total, nine people have been affected by the pneumonia, all of whom are linked to the intensive care unit of a private hospital in San Miguel de Tucumán, including one patient and eight health workers.

Testing has ruled out coronavirus, influenza, hantavirus and about 30 other possible causes. Public health officials have tested for both viruses and bacteria with no luck and are collecting DNA samples in order to eventually make a genomic identification of any virus or viral particle that can be found.

Health officials are conducting tests for toxicological causes as well.

Samples were sent to the Malbrán National Institute of Microbiology in Buenos Aires, but the tests are also returning negative there so far, said Dr. Luis Medina Ruiz, the head of the Public Health Ministry in Tucumán, during a press conference on Thursday.

Health officials in the province continue to conduct testing on blood, phlegm and urine samples for all the viruses and bacteria that they can test for.

"We are all on alert about this issue"

“We are all on alert about this issue, that is why it is essential to inform ourselves through official means, we are issuing communications to the guards and public and private intensive care units, due to the possible appearance of a patient with these characteristics, especially to make an adequate diagnosis with all the necessary protocols and PPE, as recommended for all patients with respiratory conditions," said Ruiz.

"This situation is unpredictable, in principle, more than eleven days have passed, and each patient has a different evolution, for example, one of them presents bilateral pneumonia with a significant commitment from the tomographic point of view, but you have to wait for the evolution since it is a disease whose origin is still unknown, therefore, the evolution is also unpredictable," added the Public Health Ministry head.

All of the patients have some type of comorbidities, such as smoking, COPD, a history of respiratory symptoms, obesity, diabetes or high blood pressure, according to health officials.

"We are not leaving any hypotheses unopened, we have more than thirty possible germs that we have the ability to detect and they are giving negative results, but we must also take into account that many times there may be a previous intake of antibiotics that can hide the etiology (cause of the disease)," said Ruiz.

The World Health Organization is monitoring the situation along with Argentinian health officials.

I've been watching this one this week as well- the fact it was all in the same facility, and the fact that they all had comorbidities and the fact that only one (last time I read up on it, maybe more now) 70 odd year old non health worker in the facility all seemed encouraging, and to point to it being environmentally related and not especially easy to catch if the patients are NOT acquiring it left and right.

Still, always nice when these things get resolved.

PUBLIC HEALTH September 1, 2022
The Ministry of Public Health reported on the health situation of the bilateral pneumonia outbreak
n this regard, the minister announced: “The sanatorium informed us of the death of a 70-year-old patient who was hospitalized there, we needed the family to know first to be able to communicate it. In addition, we identified 3 more employees of the private agent who began with symptoms around August 20 to 23 (that is, they correspond to the same outbreak and place of contagion in principle). That is why at this moment 3 more people are added to the 6 initial patients”.

Regarding the 70-year-old patient who died on that date, the minister stated: “She was hospitalized and underwent surgery for a gallbladder problem. She was re-operated twice. From then on, an infectious pulmonary disease took place, the date of which coincides with the onset of symptoms in the other patients, around August 20. Although it is not yet clear which came first, at first she was considered patient zero, but she is under study based on the deepening of the analyzes that are being carried out”.
At the same time he added: “We reported on the 3 identified employees, one of them is a 40-year-old pharmacy assistant and has been hospitalized a few days ago in a public hospital with a fever. In the active search that we are carrying out, especially in health personnel, we detected that it is a person who has non-severe bilateral pneumonia with an oxygen requirement under study. The other person is a 44-year-old nurse, she is at her home and we are following her evolution, since her clinic does not warrant hospitalization. The third case is a 30-year-old nurse with comorbidities, who was recently admitted to hospital this morning, symptoms began around the same days and on the date he was present to be evaluated, at which time we decided to hospitalize him.
Following this line, the official explained that these three people have bilateral pneumonia as a clinical characteristic and in common the origin of the focus, the epidemiological link with the 6 initial patients. The criterion of incorporating them as suspected patients to the outbreak in principle is based on the employment relationship with the sanatorium, bilateral pneumonia and that with all the study panels its etiology has not yet been found.
“Last night and this morning we had information from the Malbrán Institute and so far all the studies that we had as negative -for Covid, Hantavirus and some strains of Legionella- are also giving negative results there. We continue to carry out the research protocol for blood cultures, sputum cultures, urine cultures and all the viruses and bacteria that we have available in the province. We are in direct communication with Minister Carla Vizzotti, who is available, as well as all the epidemiology and infectology teams of the Nation, we are working as a large medical board in which we all participate with the information we have, there are hypotheses of the etiology that are being put on the table, although what we have specifically is the common origin and evolution of the disease, "he added.
In the same way Medina Ruiz stressed that Sanitary Control is doing an exhaustive analysis in the sanatorium and so far nothing has been found that gives suspicion of being the origin of the problem.

“We are all on alert about this issue, that is why it is essential to inform ourselves through official means, we are issuing communications to the guards and public and private intensive therapies, due to the possible appearance of a patient who has these characteristics, especially to make an adequate diagnosis. with all the necessary protocols and PPE, as recommended with all patients with respiratory conditions. If the patient had a work history from the private sanatorium, they should receive a different protocol regarding the taking of samples before starting the antibiotic scheme, so that the bacteria or germ that is causing this health problem can eventually be found” he warned.

And he continued: “We are still in the middle of the days determined for the restriction of the clinic, whether it is extended or not will depend on the epidemic situation and whether new cases appear. Isolation is complied with and from the Ministry we issue recommendations from the first moment, which are still in force, we are still in the midst of a pandemic, we continue to have patients with respiratory conditions and what is usually done when a patient is hospitalized is to investigate it , Covid, flu etiology is ruled out, based on that it is determined where the patient is hospitalized and in these cases there is a third differential isolation, with a differential human resource ”.
Continuing Medina Ruiz explained that all possible etiologies are being evaluated and even capturing DNA samples to eventually make a genomic identification of any virus or viral particle that can be found. At the toxicological level, in addition, samples are being studied since they do not want to leave any hypothesis out.

“Our teams are going to the clinic offering support to do an examination of the patients identified according to the pathology and treatment. Although the private clinic has its autonomy and responsibility carried out by its management, we have offered to support it with resources and advice, ”he explained.

Likewise, the official emphasized that there is no patient who has an epidemiological link with the sick. In any case, he advises the population to continue with the care. “The most important thing is that if someone had a working relationship during those days with these patients and presents any symptoms, they present themselves at our on-call services and report it. This situation is unpredictable, in principle more than eleven days have passed, each patient has a different evolution, for example, one of them presents a bilateral pneumonia with an important compromise from the tomographic point of view, but we have to wait for the evolution that it is a disease whose origin is still unknown, therefore, the evolution is also unpredictable”, he stated.
“Regarding the close contact group, the personnel who were sharing work space, which were around twenty people, are being closely followed. In principle, all patients have some type of comorbidity, such as smoking, COPD, a history of respiratory symptoms, obesity, diabetes, and high blood pressure. We do not leave any hypotheses closed, we have more than thirty possible germs with the ability to detect them and they are giving negative results, but we must also take into account that many times there may be a previous intake of antibiotics that can hide the etiology, "he said.
It should be noted that first a meeting was held in the Ministry's office with the committee of experts and managers, infectologists and epidemiologists from the Centro de Salud, Eva Perón and Ángel C. Padilla hospitals. “The committee of experts with whom we are working and concluding that it is an outbreak that has to do with the sanatorium, that in one or two moments he was exposed to the germ or bacteria, and that from that the infections began since after the first exposure around August 20, there were no new infections either in the family or with work contacts,” he concluded.

The Medical Executive Secretary of Siprosa, Dr. Miguel Ferre Contreras , the Director of Health Management, Dr. Alejandra Mattiacci , the Director of Epidemiology of the Provincial Health System, Dr. Rogelio Calli , were present

San Miguel de Tucumán "mystery pneumonia" outbreak, what we know and don't know about it.

Since this caught global attention today, I'm going to summarize the facts we know so far.

The Public Health Minister reported the event on AUG 30th:

2/

"We considered COVID and Influenza, both A and B; even Hantavirus, but through some research they have been ruled out".

They also considered some extra bacteria and viruses through FilmArray tech.

So, I assume they used the BIOFIRE FILMARRAY Respiratory 2.1 plus Panel: Image

3/ Therefore, all the pathogens listed above + Hantavirus, have already been ruled out.

What abt the cases?

2 of them were hospitalized in a private medical center
2 more in a public hospital
1 woman was isolated at home

The hospitalized required Mechanical Ventilation.

4/ On AUG 31st, the Ministry of Health reported "the death of one of the patients linked to the pneumonia outbreak, whose etiology is under investigation".

msptucuman.gov.ar/comunicado-ofi…

He was a 45yo patient with comorbidities, hospitalized since AUG 21st.

5/ The same day (AUG 31st), it was reported that The National Administration of Laboratories and Health Institutes (ANLIS), also know as "El Malbrán", a highly reputed institution in Argentina, received samples from the confirmed cases, in order to execute extended analyses. Image

6/ Now, it was publicly known that the total cases consisted of 5 health workers and 1 private patient.

Their symptoms began between AUG 18th - AUG 22nd.

The total number of deaths was elevated to 2.

7/

Follow Up / Tracing activities were since conducted by health officers in order to detect possible new linked cases.

Important: No new cases were discovered until AUG 31st.

So, 3 HW were still hospitalized, and 1 more (the woman) was still isolated at home.

8/ Initially, the name of the medical center were the outbreak took place, was not disclosed.

But now we know that it was the "Sanatorium Luz Médica".

We even know that it was isolated/shut down for 7 days.

Only high risk patients could keep medical consultations there. Image

9/ The Sanatorium is located right in the middle of the 1.5m population city of San Miguel de Tucumán.

10/ SEP 1st, 2022:

Huge news here. The Ministry of Public Health reported 3 more cases today. and the death of a 70yo woman.

Their symptoms began between AUG 20th - AUG 23rd.

No. of active cases: 7
No. of deaths: 3

The woman was considered "patient zero" at first. Image

11/ But the health officers team is still investigating in order to confirm/deny this initial thought abt her.

The identity of the 3 new cases was also disclosed:

All of them HCW.
a 40yo pharmacy assistant.
a 44yo nurse (isolated at home).
a 30yo male nurse w/comorbidities.

12/ Important: It was also reported that yday "El Malbrán" shared some results w/the local authorities:

"So far all the analyses that we had as negative -for Covid, Hantavirus and some strains of Legionella- are also giving negative results there".

i.e. a double confirmation.

13/ Another important piece of info:

"Sanitary Control is doing an exhaustive analysis in the sanatorium and so far, nothing has been found that gives suspicion of being the origin of the problem".

They are even "capturing DNA samples to identify any virus or viral particle".

14/ But there are some good news too:

There are no new infections either in the family or with work-related contacts. All the cases share the same link so far.

-

So, what kind of pathogen is still considered as the main cause of this event?

There are is ONLY 1, afaik. Image

15/ Since yday, local media and popular opinion are pushing a narrative: They think this was the "Legionnaires' disease", caused by Legionella bacteria.

BUT, remember that earlier today, the Health Officers already ruled out that one too (video below):

16/ There are many species of Legionella, but the most popular is Legionella pneumophila.

We don't know which species of Legionella have already been ruled out, but even if pneumophila was discarded (Im sure they did), there could still be a very small chance of being Legionella Image

17/ Apart from that, there is the only 1 pathogen that I was talking abt some tweets above:

Leptospira interrogans.

AFAIK, the argentinian team (including "El Malbrán") are still considering this bacteria, i.e. it hasn't been ruled out yet.

Could this be then Leptospirosis?
Image

18/

Argentinian local authorities are doing all they can to identify the pathogen or cause behind this potentially serious event.

Just please keep calm, and wait. Don't repeat the mistakes that we made with the COVID pandemic, and hope for the best.

Still waiting for more info from Tucumán health officers. Yday, they clearly declared that this was NOT SRAS-CoV-2. 25 more bacteria and 1 hantavirus were also discarded. Only 1 woman had "Mild Symptoms". The rest of the cases are in very serious condition or dead.
@RolandBakerIII FYI
Important to note: Yday, mass media from Argentina considered Legionella as the most probable cause. Today, Tucumán health officers stated that Legionella is discarded as such lagaceta.com.ar/nota/959355/so…

Argentina reports 1 new case of unidentified illness

Health authorities in northern Argentina have reported one new case of an unidentified illness that has sickened at least 10 people, officials say. The cause of the outbreak remains unknown.
The health ministry in Tucumán province said in a statement on Friday that an 81-year-old man with comorbidities has been linked to the outbreak. The man is hospitalized in the public sector in serious condition.

The statement did not specify where or when the man may have been infected. All of the prior cases had been found at Luz Médica, a private clinic in the city of San Miguel de Tucuman.
Dr. Luis Medina Ruiz, the region’s health minister, said at a briefing on Thursday that experts have carried out tests for more than 30 possible causes so far – including coronavirus, influenza, hantavirus, and multiple strains of Legionella – but all have come back negative.
The first known case is a 70-year-old woman who was admitted to Luz Médica in mid-August to undergo surgery for a gallbladder problem. She developed a lung infection on August 18, after which healthcare workers who had contact with her started falling ill.

A doctor and a nurse later died, and the first patient died on Thursday morning. Four other patients, including the new case, remain in serious condition. Three more are being treated on an outpatient basis. Symptoms of the illness include shortness of breath, pneumonia and fever.
“We are all on alert about this issue,” Medina Ruiz said. “We are releasing guides to public and private intensive care units for the possible appearance of a patient with these characteristics, especially to make a proper diagnosis with all the necessary protocols and PPE, as recommended with all patients with respiratory conditions.”
The World Health Organization (WHO) and the European Center for Disease Control (ECDC) are both monitoring the outbreak.

It's the OR at the original clinic.

Check the OR. Check the autoclave water is being changed or burned through and replaced EVERY time.

This same thing happens on cruise ships but usually doesn't kill people. It's the re-infection (sound familiar? ) of the weakest among us that is troubling.

Bilateral pneumonia of unknown origin: how research is progressing
The Minister of Health, Luis Medina Ruiz, provided details of how the search for the cause of the disease that killed 3 people in the province continues and what will happen with the isolation of the sanatorium where the infections began.

At mid-morning this Friday, the Minister of Health, Luis Medina Ruiz , gave a press conference to talk about the progress in the investigation of bilateral pneumonia of unknown origin , which caused the death of three patients and has Tucumán and the country. , in suspense.

"With the Malbrán Institute last night we held a virtual meeting together with the Nation's Minister of Health, Carla Vizzotti, and national and international references in this type of case, who confirmed to us that the studies that are being carried out now are being ratified. the negatives that we had," said the leader of the provincial health portfolio.

And he explained: "In any case, there are studies that involve cultures and that takes more time. In addition, when a study is negative, another one is sought that is more sensitive and more complex that takes more time."

Medina Ruiz also reported that there are "alerts in our on-call services, in which any patient who has an epidemiological link or who is a health worker in this private sanatorium and has a respiratory condition, enters into a suspicion protocol where investigation is done with tomographies and laboratories to rule out or confirm that it can enter the group of infected ".

Regarding Medical Light, the official added that "19 patients remain hospitalized and a team from the Ministry is evaluating the situation of each one of them."

"The time to detect patients is from 7 to 14 days. The isolation of the sanatorium will be extended or not based on whether new patients appear," said Medina Ruiz.

This is not good. He's not at the same hospital as the other 9 cases. Is he related (in a contact way) to any of the other cases or was he at the private hospital any time recently - or is this a totally isolated case? We need more info...

Lepto cases in dogs and raccoons are really high this year in the states, so this wouldn't surprise me in the least. Though I don't know if they have raccoons in Argentina, they do have plenty of other rodent type critters that can spread the disease.

Ten years here locally in central Iowa, several Iowans became ill and IIRC one elderly person died, when they contracted lepto from their infected dogs. This is another reason why Jr. is vaccinated for it every year, since he gets walked twice a day.

Yes, but lepto hasn't ever been known as a respiratory virus... it generally attacks the kidneys, and causes late term abortions in cows, and a secondary pneumonia isn't uncommon due to the damage to the organs and immune system. I wonder if some overeducated moron has been playing in the lab again?

This is what I'm wondering as well... I can see pneumonia happening in the elderly, or those with a severely compromised immune system.

I asked my boss /vet about Lepto in humans this afternoon

And the answer I was given which was “I do not deal with humans, don’t ask me”… which is his usual response

However.
In my opinion, and from personal experience…this ain’t Lepto
I also asked another Vet in our practice who was very alarmed at this new outbreak…she also would not answer because she has no idea what this could be. She “thought” that pneumonia isn’t a common Lepto infection in humans.

Years ago we had a client become infected with Lepto from her dog, which we we had in our care. Before we got a definitive diagnosis the owner was already sick, and in the hospital where they found Lepto as the cause of her illness. The diagnosis didn’t take long either. The hospital was ON it.
Lepto is spread by urine, water, feces…mud puddles… this is why I ALWAYS get on clients if they try to refuse the DALPP vaccine to save money!!! Omg!
Lepto, Etc are all contagious (zoonotic) to humans! It causes severe illness and kidney failure if not treated asap.

Our sick sick dog and owner both recovered but it was a long week of constant care, meds, and daily IV fluids for the dog. We had strict instructions to not touch that dog without gloves and a mask.

So.
Im not a specialist OR a doctor, obviously, but I think they’re “reaching” out of cold, stone fear.

If this IS Lepto, well, I’ll be glad to apologize for my ignorance.

Otherwise…. We could be looking at a new , hideous virus that has a high death rate and I’m not ready for that.
