Remote Paramedical pain in the ass
At the Hospital

Ivar spent the day and long into the night sitting with Sofia. She was still heavily drugged to keep the pain under control and to keep her from thrashing around until she knew the extent of what was going on. When she was awake, they talked, mostly about nonsense. When she was asleep, Ivar sat and read through some books on his electronic pad. He wasn’t really getting anything out of the reading though.

Several times through the day Deloris stopped by and chatted. Through these little talks he learned she was divorced, been a nurse since forever, and had moved into the top floor of the hospital along with the few people still working here pretending things were normal-ish. They weren’t getting paid anymore other than room and board but compared to the cost of food and trying to get fuel to drive back to her apartment, it was effectively a raise.

As they talked, Ivar started expanding on his and Amber’s idea, feeling her out to see how attached to the area she was. Turns out she wasn’t. He could see the uncertainty and a little kernel of fear as they talked about how things were headed and where it might end up. Ivar told her about why it was so important they get Sofia able to travel soon and a rough idea of where they were going and why. He was glad she didn’t automatically act like they were nuts and worried too much. He knew the seed was planted. When Amber came tomorrow, he would have her broach the idea to her of coming along with them.

I'm not one who should be knit picking about spelling but I think the word you were looking is "thrashing" and not "trashing". The "H" key is sneaky sometimes.

Petra had Jennifer working with her as she finished connecting the spare batteries they had into charging systems in the various campers. They would try to not run generators or idle the trucks for power when they stopped on the trip north. With the extra batteries she was installing, as long as they didn’t try to run too much stuff, they should be fine. Twelve-volt lights and space heaters or electric blankets would keep the draw on the batteries low, leaving them with plenty of reserve.

They already had a lot of the stuff they were taking either packed into the campers or in boxes on the trailers. All the extended tanks on the tow vehicles were as topped off as they could make them and the transfer tanks in the bed of some of the trucks and trailers were as full as they could manage.

Silas and Jonas were distributing the leftover weapons not already assigned and set up for people along with the small stockpile of ammo not already packed. They wanted to make sure if something happened to a vehicle, they could make up the shortfall without too much effort.

Amber was trying to figure out where the best place Sofia could ride. She still didn’t know how they were going to be able to do this. Moving someone around like they will have to for the trip north after such a serious surgery is not a good idea, but neither is staying here.

Amber dug into her reference books for the post-op dangers and there were a ton for her to worry about. Pulmonary and cardiac complications were two of the biggest but were nowhere near the only ones. Venous thromboembolism, transient ischemic attack, stroke, renal failure, septic shock, pneumonia, pulmonary edema and the list goes on and on. She didn’t know what she could do about most of them without a hospital. This was the part that scared the shit out of her.

All of this had Amber looking at all four campers, trying to figure out what might work so she could plan for it and begin pre-positioning the medical gear she might need if it all goes wrong.

Ivar was tired. Interrupted off and on sleep schedules weren’t that bad at twenty-three, but start being a bit harsh after forty. He was only in a light sleep, trying to be ready for anything Sofia needed. She was doing a little better the last two or three times she woke up. She was able to talk a few minutes haltingly before the next round of drugs kicked in. He told her she had to rest and conserve her energy for healing. It tore into him to look into her eyes and see the pain and confusion he couldn’t fix for her right now but he was there for her.

He woke up. Something was wrong. It was quiet. Really ****ing quiet. He tried to hear or feel what was wrong before he moved. Next to him, Sofia was sleeping. He could feel her breathing. He tried to hear other things.

In a busy hospital, there was always sound. Well, this hospital wasn’t that busy anymore to begin with. They didn’t have many in-patients. Most everything was operating like a clinic; people would come here, get stuff fixed and leave. All the ones staying overnight were here in what was the ICU and Step-Down wing on the floor with the ORs.

Deloris had given him the lay of the land as they talked earlier. As she described how things have been changing, he kept seeing in his memory all those clinics and shoestring hospitals in third world countries he had been to; dwindling supplies, reusing what was once disposable medical gear, unsuitable substitutions when it came to medicines. All of this just gave more credence to his offer. When she checked in on them right before bedtime, she told him she was seriously considering saying yes.

Ivar lay there another five minutes trying to figure out what was going on before he got up to investigate. Finally, he knew it was time to start checking things out.

Out from under the covers, the first thing that struck him was the cold. The room wasn’t that warm earlier and now it was a lot worse. He took the second blanket and put it over Sofia’s sleeping form.

He slipped into his boots. He saw Die Hard too many times. He wasn’t going investigating without shoes. His jacket covered his pistol and the rest of the stuff he had on his belt. The staff, or at least the staff in this hallway and the security guys, knew him but he didn’t want to attract more attention than he had too.

Flashlight in his hand, he slipped out of the room, looking for answers.

I think Deloris should take something in lieu of back wages, like a fully equipped ambulance. Of course, CCG seems to be heading that way anyway. The big question is if Deloris is thinking of taking someone with her?

Deloris should deliver her to the "compound" in a fully equipped ALS bus! Might even invite an intern friend along.
Thank you. Doesn't seem like there's much time left before the exodus. And, the Pixie is still fully dressed :).

Love your thinking but there really isn't that much in a 'fully equipped' ambulance. The best bang for the buck is the cardiac monitor, jump kit and the drug box. The rest is xx 4x4 gauze, xx bags of saline, backboard, headblocks, stretcher, o2 tank, rolls of tape and so on and so forth.

Looking for Answers

There wasn’t much movement in the hallway. ‘Could be because it was dark as shit!’ Ivar thought to himself. He had a couple levels of illumination available on his flashlight. He picked the ‘barely a glow’ one both to conserve the battery and to keep most of his dark adaptation.

He kicked himself for not having his night vision goggles in his bag. What was the point of Paige squirreling away all the goodies if he didn’t use them? Silas will dog the shit out of him if he finds out.

Ivar headed for the nurse’s station. Where was everyone? He knew there was only Sofia and two others here right now, but there should be some nurses around, or some family, or somebody!

The nurse's station was empty.

Ivar looked at the charts on the rack at the desk. Finding the room numbers for the other two, he went forward to investigate.

The first room he came to was empty as well but there were signs of hasty departure. He moved to the next one. Here he found people.

There were two nurses standing next to a bed, talking to a crying family member. Ivar didn’t want to get in the middle of whatever this was, but he needed info so he stood quietly at the door, trying to not be noticed.

“…I know it’s hard Mrs Johnson, but your husband was already on a ventilator and the prognosis wasn’t good. We tried to keep him going when the power went out but liked we talked this morning, it was close to the end already.”

Ivar saw the nurses were worn out. He could see the resuscitation bag still hooked up to the tube in the patient on the bed. There were empty drug boxes on the bed and the floor. The cart next to the bed looked like a five-year-old’s toy box after a hard day of playing.

These girls had worked their asses off trying to save the guy in the bed, even though, from the sound of it, he was already terminal. They weren’t much older than Petra. Damn! Yet another event to remind him of why he could never work in medicine.

One of the nurses saw him and came over to where he was standing.

“Everything alright, sir?” she asked softly.

“Fine, I think, except for the power. Where’s Deloris?”

“Yeah, the power is a problem. The back-ups normally kick on a lot sooner. Delores said to let you guys rest. She had to run an errand in another part of the hospital. She said she would be right back.”

The nurse exuded false confidence. Ivar could tell. He had seen it enough times from some of the young troops on their first deployments. He knew she was trying to make him not panic. He couldn’t let on that he knew she was close to panic herself.

“Well, I’ll head back to Sofia’s room in a moment, since you say things are alright. Let Deloris know if you see her, ok?”

“Sure thing, don’t worry sir.” The nurse turned back to help the other nurse with the grieving family.

Ivar slipped out and continued his search for answers.

There was something in the hallway out of place. As he got closer, he could see it was a wheelchair with something in it. When he got to it, he saw it was a sheet wrapped around something and it took up the whole chair.

He moved past it and was about to turn the corner when Deloris came around the corner with another sheet-wrapped bundle. As soon as Deloris saw him, she thrust the bundle into his arms.

“Here. Take this and the other one in the wheelchair down to Sofia’s room. We need to get her into the chair and out of here. I’ll meet you in the room in a few minutes with another cart we can put the bundles on.”

Ivar started to say something, but she was already gone, back down the hallway.

He took the bundle and dropped it onto the one in the wheelchair and headed back to Sofia’s room.

Once there, he emptied the wheelchair and moved it next to the bed.

“Sofia, Sofia…You need to wake up, baby. It’s time to go. I need your help. We need to get you into the wheelchair.”

Ivar knew Sofia was still out of it. She was hardly there at all. He got her sat up enough to get the gown off of her. He wasn’t taking her out of here in a hospital gown. He knew he could get some underwear on her and a pair of summer shorts. He dug around in his bag and found his neck gaiter. This he pulled up over her non-broken leg to cover at least some of it for warmth.

Her top half was easier. He had a t-shirt and a fleece pullover. Her heavy coat was in the SUV so she should be good. He locked the wheels and started the most delicate part. Sofia wasn’t by any stretch of the imagination a tiny woman. At nearly six feet, and built well, she wasn’t light. He was scared to death he might drop her and hurt her moving her to the chair.

The chair was bigger than he expected. This helped, giving him to maneuver her. Once she was into the chair, he struggled with getting the leg supports up, finally succeeding. Another decision came to him. If he was wheeling her out of here, he was covering her up with a couple of the blankets.

He just finished swaddling her in the third blanket when the door to the room opened. It was Deloris pushing a laundry cart.

“Great! You already have her in the chair!” Deloris said as she grabbed the other bundles and dropped them into the laundry cart.

“So, you going to tell me what the hell is going on?” Ivar wanted to know, wanted to know about potential threats.

“Power’s done, not coming back and this place is going to turn into an icebox by dawn. The hospital isn’t going to be of any use anymore.”

“So, it’s time to go.”

“Yup. I got my stuff from my room and some supplies we will need for Sofia.”

“OK, sounds good but we are three floors up. How do we get down? Will I have to carry her, then how do we get the rest of the stuff in the cart down? I’m sure you have a plan.” Ivar said as he pushed Sofia’s chair following Deloris as she pushed the laundry cart.

“Well, as long as my plan works, we will be able to take the elevator for one more ride.”

“You said the power was out. Elevators need power.”

“They do, but the freight elevator in the old section is a hydraulic one. As long as there is enough battery back-up to read the sensors and not lower the elevator until the doors are closed, we will be good.”

“I don’t follow you. Say again?”

“The first time the power blipped last night, I called the old freight elevator and blocked the doors open. The normal power out response for that elevator is a slow release of the hydraulics, lowering the car to the ground floor. So, if it works, we get to take the elevator down.”

“Makes sense. Hope it works.”

At the House

Silas, Amber, Jonas and Petra were sitting around the dinner table late in the night after Jennifer went to bed discussing the ramifications of Sofia’s injury and cast.

“Well, there just isn’t a good and safe way for her to climb into the cabover bed in their camper unless we wait a week or two after she gets back to the house for her to get stable enough to try.’ Amber concluded.

“And you say it will be a week before she even comes to the house?” said Silas for clarification.

“Yeah, probably a week. There is so much that can go wrong with a major surgery like she had. Pulmonary and cardiac complications are two of the biggest things I worry about. Venous thromboembolism, transient ischemic attack, stroke, renal failure, septic shock, pneumonia, pulmonary edema and the list goes on and on. The big problem is if any of those show up, most of them I can’t do anything about here. We need to get her stable and then move her. I don’t want to be in the middle of nowhere and suddenly need to fix a pulmonary embolism or something a lot worse. Sabine left a great medical clinic set up but it wasn’t built for stuff like that.”

Petra surprised herself by speaking up.

“I hear a lot of problems. What are the solutions? Do we stay here another month or two until she can travel? Is that where you are headed with all of this?”

“That is one option I was considering…” Amber was cut off by Silas.

“We need another option, another plan. Don’t forget why we have this problem. We don’t know if there are any more going to pop up here with the same idea. If Mickelson could find us, others can too. We need to get moving north. There is no paper trail leading anyone looking for us there unless we screw up and leave a map book with all the notes and such. We have to leave here to be safe.”

“I agree.” Jonas added. “We need a plan for how to get out of here safely as soon as we can.”

Amber took a deep breath before she pitched her other idea. She wanted to wait a couple days so everyone could have a buy in on the plan, especially Ivar and Sofia but she may as well lay it out now.

“Well, I looked at the measurements, the heights and such. Your guy’s trailer is the one best suited for Sofia to be in. You have four bed setups in there. If you can adjust to let her have one of the bottom beds, it would be easiest to get her into and out of.”

“Then it also makes sense for her to ride in the back seat of the crew cab as we drive. Jennifer could keep an eye on her and help with her while I drive.” Jonas wasn’t too disturbed by the idea. With everything Sofia and everyone else had done for them…Hell they wouldn’t have the truck or trailer without them, much less somewhere to go. Making room for Sofia in their trailer wasn’t much to ask. You did things like that for friends.

“If you needed more room, Jennifer could bunk in mine with me. She could have the upper bunk and I just flop the futon back into a bed, no problem.” Petra said quickly, wanting to be helpful, and to cover up the part of her glad it wasn’t her trailer being chosen for sickbay.

“Might not be a bad idea since Ivar will probably want to be there to be close to Sofia since he won’t be with her during the driving.” Amber thought out loud.

"Well, we have some time to get it all figured, but we should have decisions made by the time she gets released from the hospital.” Silas said to the group.

“I’ll head down there in the morning to check on her and let Ivar and Sofia about the ideas we came up with. That means I need to go to bed.” Amber said as she stood up. This set off a cascade effect with everyone getting up and heading to bed.

Petra was the last one to stand up. A corner of her brain was messing with her. ‘You know, if Jennifer was in your trailer, Jonas would want to be nearby. If the two of you can fit on the bench seat in the back of the truck, then the futon is plenty huge enough for the two of you. You could keep him warm…’

Petra took the time to wash out cups sitting in the sink. This gave her time for the blush to subside and the voice in her head to shut up.
