Had a patient show up in the ED complaining of abdominal pain. Patient was suspected to have had lesions develop after a surgical procedure about eight weeks ago. It has now become common practice to test all patients for Covid-19 if they are being admitted to the hospital, no matter what the symptoms. Even an impacted tooth, if admitted, will be tested in order to send them to the 'correct' unit and limit spread in the hospital.

The patient with abdominal pain had ZERO signs and symptoms of Covid-19 was found to be positive for the virus. There ARE asymptomatic carriers everywhere.


I well believe there are asymptomatic carriers, probably everywhere. I've said that the US was "Well Seeded" several times on this thread, asymptomatic carriers are likely a major contributor to that effect.

Just making note that the data from the CCP was suspect in every way.

This is my take the COVID 19 virus spread prodigiously prior to any lockdowns and probably equally so during lockdowns, it is very contagious and even during lockdowns there is considerable contact between people. IMO there is already considerable infection within the population which does not mean we're all going to die, just that we were all infected earlier than normally thought.

Many of the studies tend to agree with you; the main vector seems to be the families, or units of folk living together...

My opinion is that most people have not been infected, say 10 percent out of the herd 80%. In Spain and Iran it is 15 %,. The reason the usa has 500,000 cases is specific leaders took specific actions, in specific places, like New York city etc, Now we are starting to see a massive wave of people being infected but that comes from specific groups, like the college beach hordes, or from europe on the case of New York. We are BARELY AT THE END OF THE BEGINING HERE IN CONUS. CUOMO SUTE SHUT UP TODAY. I told you all don't bother with New York city till they have 25,000 dead. We have down everything wrong with corona chan and we will end up with between 100,000 to 300,000 dead in CONUS if my read is on, but if not it will be 1 to 3 MILLION DEAD.

Update: California Gov. Gavin Newsom and other officials were providing updates on the state's response to COVID-19. Motel vouchers for health care workers discounts scaled to pay. Helath care workers who have tested positive and those exposed also can access hotel rooms.

18309 tested positive, 492 deaths (50 yesterday,) 1132 in ICU (drop of 1.9% from yesterday,) 2825 hospitalized. We have bent the curve but it as been stretched so will peak later. We started with 7587 ventilators, now have 11747 ventilators. Hospitals only using 31.89% of their ventilators. (8,000 not in use) This does not include the CA state stockpile. That is why CA was able to lend ventilators to other states (There is a claw back agreement if needed.) They check in daily with every county on what they have and need. They make sure they have 2X as many vents as they need that day and stockpile are regionally prepostitioned and hospitals. Have mutual aid system. Taking care of the smaller hospitals where surge could overwhelm. Goal is to secure 10,000 more ventilators. Bridge ventilators (not built to last forever, step down for those not at same stage of acuity as the regular.)

Capacity of CA to procure hundred of millions of units PPE items for hospital and l system, not include grocery front line and IHSS (In home health service) hundreds of thousands pushed out daily and will increase significantly over next few weeks.

Japan to distribute masks nationwide next week

The Japanese government will start mailing reusable cloth masks to all households in the country next week in a bid to address the continuing shortage.

Households in Tokyo and other urban areas will be the first to receive the masks as they have the largest number of coronavirus cases.

A total of 58.5 million households will each receive two cloth masks. The health ministry says the packing is now underway and a leaflet explaining how to wash the masks will be enclosed.

The total cost will be 430 million dollars, including postage. Half the money will be covered by reserve funds and the other half will be included in a supplementary budget for the current fiscal year.

The ministry also plans to deliver 130 million cloth masks to elderly people in care facilities and to pregnant women after the passage of the supplementary budget.

Ivermectin is NOT a cure for COVID-19 (At least not yet)

Ivermectin is an FDA approved medication for parasitic infections. Ivermectin is widely available, due to its inclusion on the WHO model list of essential medicines. It opens glutamate-sensitive chloride channel currents in helminths, and its widely believed that this is how it kills certain parasites. Its the drug of choice for the treatment of onchocerciasis and strongyloidiasis. So why might Ivermectin work against SARS-CoV-2?

Once the virus gains entry into the cells of the body, using the ACE2 receptor, the virus releases its messenger RNA into the cell. The cell starts to translate the mRNA from the virus, which starts to make viral proteins, and that’s how new virus forms.

Some of these viral proteins that are freshly made then enter into the nucleus of the cell, or are at least thought to do so by many scientists, and this probably impairs our cells' ability to fight off the infection. The way that these viral proteins are thought to gain entry into the nucleus of our cells is through specific channels that are made out of a different protein, called importin. Not important, importin. There's importin type alpha, importin beta. Or Imp for short, so imp alpha, imp beta. And this is where ivermectin comes in, because ivermectin inhibits passage of these viral proteins into the nucleus. It does so by binding to these protein channels, and effectively blocking the transport of molecules through them. So this is why scientists started studying ivermectin.

And this recent article that just came out April 3rd, in the journal Antiviral Research. This is pre-proof article, meaning they have been peer reviewed, but final publication is still pending. What these scientists found is that ivermectin significantly inhibits the SARS-2-CoV viral proteins in their passage from cytoplasm into the nucleus of cells…... in vitro.

In vitro means this was done in a test tube, or outside of a living organism. A single dose of Ivermectin had ∼5000-fold reduction in virus levels at 48h in cell culture. So this drug has a huge impact on this virus in vitro. And that is why the authors of this study conclude that these drugs needs to be studied in COVID-19 patients, meaning in clinical trials. The same way that other drugs are being looked at in clinical trials for COVID-19, such as remdesivir, chloroquine, and hydroxychloroquine.

So because Ivermectin is generally considered a safe drug and is already FDA approved for other diseases, it can be repurposed for COVID-19, as long as it is shown to be safe and effective for COVID-19 patients in clinical trials.

If this drug turns out to be the miracle drug for COVID-19, the next question will become, when should people start taking it? Because for the patients with COVID-19 who get very sick, they usually get sick very quickly. So this ivermectin may have to be given prophylactically, or at the first onset of symptoms.

The other question, is what are the side effects, or adverse reactions? Well, base on what we already know about ivermectin, this is what I can tell you. The most common adverse reaction is the Mazzotti reaction. But this only happens when ivermectin is used to treat onchocerciasis, a parasitic infection. The Mazzotti reaction was first described in 1948. It’s a symptom complex seen in patients after undergoing treatment of nematode infestation, particularly with the medication diethylcarbamazine (DEC), but with ivermectin as well.

Mazzotti reactions can be life-threatening and are characterized by fever, hives, generalized swelling, swollen lymph nodes, fast heart rate, low blood pressure, hypotension, joint pain, and abdominal pain.

There is a 25% chance of Mazzotti reaction when ivermectin is used to treat onchocerciasis, but not other parasitic infections. So if a COVID19 patient took ivermectin, this would not happen.

So, otherwise, Ivermectin is very safe, and side effects are uncommon. Diarrhea occurs in 2% of people taking it.

Also, Ivermectin should not be administered to pregnant or lactating women, and its safety in children less than 15 kg is not known.

Former Japan official: COVID-19 cases likely higher than reported

April 9 (UPI) -- A former Japanese health chief says the number of coronavirus cases in his country could be 10 times higher than official estimates, citing the low number of reported infections.

Yoichi Masuzoe, a former health minister and ex-governor of Tokyo, said the administration of Prime Minister Shinzo Abe delayed taking action and did not heed Masuzoe's warnings, local newspaper Mainichi Shimbun reported Thursday.

Masuzoe said he urged the government to make polymerase chain reaction testing more widely available to the public, but Tokyo did little in response.

The former health chief added he could not rule out the possibility the total number of cases could be 10 times higher, or "by another zero," according to the report.

Masuzoe's criticism of the government comes at a time when Japan is reporting a spike in cases. The number of cases, fewer than 5,000, remains relatively low compared to other countries. The statistics do not include patients infected on the cruise ship that was docked in Yokohama in January.

As more patients are being confirmed for COVID-19, data is becoming available about the infected population in Japan.

According to Chief Cabinet Secretary Yoshihide Suga, 70 percent of patients who died from the disease are men, while 60 percent of all cases are also men, NHK and Jiji Press reported Thursday.

The numbers roughly align with statistics from the World Health Organization. According to the WHO, about two-thirds of patients in Europe who died from COVID-19 were men.

On Thursday, Suga also said underlying conditions and age had an impact on the severity of the infection.

Japan did not declare a state of emergency until after the 2020 Summer Olympics were postponed on March 24.

The Daily Beast reported Thursday Japan's low level of testing makes it "difficult to accurately assess the COVID-19 prevalence rate."

Analysts surmise the actual number of cases and deaths could be buried in Japan's pneumonia statistics, according to the report.

Japan says it has conducted 61,498 tests, far below neighboring South Korea at 494,711. The two countries experienced their first outbreaks at a similar point in time.

So, is it effective? Hydroxychloroquine is an anti-malarial drug that has been reported to be effective against the symptoms of the Wuhan coronavirus. Yet, Dr. Anthony Fauci, the director of the National Institute of Allergy and Infectious Diseases and one of President Trump’s top officials working to curb the outbreak in the U.S., has warned against saying this is full proof. He’s a professional. This is a new virus. This drug has not been tested in this regard, but thousands of doctors are saying it works.

In Michigan, Democratic State Representative Karen Whitsett contracted the disease and was given the drug. She said she felt relief from her symptoms in less than two hours. She was aware of hydroxychloroquine due to a recent Lyme’s Disease diagnosis but didn’t know it could be used to treat COVID-19. She credits the drug and President Trump for possibly saving her life.

Jackson and Coker, a healthcare staffing firm, did a survey of 1,200 doctors nationwide and found that 65 percent would prescribe the drug to their family if they were infected with the Wuhan coronavirus, with 67 percent saying they would take the drug themselves:
Sixty-five percent of physicians across the United States said they would prescribe the anti-malaria drugs chloroquine or hydroxychloroquine to treat or prevent COVID- 19 in a family member, according to a new survey released today by Jackson & Coker, one of the country’s largest physician staffing firms.
Only 11 percent said they would not use the drug at all.
Meanwhile, 30 percent of the surveyed doctors said they would prescribe the medications to a family member prior to the onset of symptoms if they had been exposed to COVID-19, a highly contagious virus that causes a pneumonia-like infection of the lungs.
“Working in healthcare, we’ve learned the best way to get a candid perspective on treatment options from a physician is to ask: ‘Would you give this to your family?’” said Tim Fischer, President of Jackson & Coker. “Families across the U.S. – and the world really – want to know what they can do to protect and save their loved ones.”
Jackson & Coker conducted the survey of 1,271 physicians from 50 states from April 4 to April 7. It conducted the survey not to influence the debate in treating patients with anti-malarials but to make sure the voice of physicians is represented. It has a margin of error of +/- 3 percent with a 95 percent confidence level of the doctors surveyed.
Maybe in a few more weeks, there can be a definitive declaration that this drug is effective, but there is story after story showing that this drug works. It could be a nice stopgap until we have an official vaccine.

It's turned out to be considerably less lethal over the entire population than early reports led us to believe. But given the high ratio of infections, the death toll will still be massive and the hospitals will be swamped for some time yet.
