World leaders rehearse for a pandemic that will come 'sooner than we expect'

The government ministers were facing a new infectious disease outbreak. The mysterious virus was sickening and killing people with alarming speed. Some patients had to be placed on ventilators to help them breathe. The new virus seemed resistant to antibiotics and antiviral medicine.

Within a week, officials had closed a major hospital and schools and quarantined thousands of people. Fear and panic spread quickly as people in neighboring countries became infected and died.

That scenario was part of a pandemic simulation held during the World Bank's annual meeting in Washington this month. It's not the kind of event that people would typically associate with the World Bank. But it's the fourth such exercise the bank has helped organize in the past year, reflecting what experts say is the growing awareness outside the traditional global health sector of the increasing threat and economic disruption posed by a global pandemic.

The chaotic and "horrendously inefficient" early response to the 2014 Ebola epidemic in West Africa was the catalyst for the simulations, said Tim Evans, senior director for health, nutrition and population at the World Bank.

"We realized that people were just making it up as they were going along, including us," Evans said, referring to the Ebola response. The bank wanted to "move from a history of panic and neglect to one where we're going to start to prepare much more systematically to be ready for the 100 percent probability we will be dealing with this again," he said. "Probably sooner than we expect."

Outbreaks of life-threatening infectious diseases are spreading faster and with more unpredictability than ever.

An unusually large plague outbreak in Madagascar has killed 106 people since August. About 70 percent of the cases are the more virulent form of pneumonic plague that spreads by coughing, sneezing or spitting and is almost always fatal if untreated with antibiotics.

In Uganda, officials are on high alert because of a recently reported outbreak of the deadly Marburg virus that has killed one person and may have exposed hundreds more at health facilities and during traditional burial ceremonies. Marburg is a highly infectious hemorrhagic fever similar to Ebola and is among the most virulent pathogens known to infect humans.

For the World Bank simulation, organizers looked at the impact on travel and tourism of an outbreak of a mysterious respiratory virus in a hypothetical country. Participants included finance, health and tourism ministers from about a dozen countries, and officials from organizations including the World Health Organization, the Centers for Disease Control and Prevention, and the International Air Transport Association.

Discussions during the 90-minute session were off the record. But in interviews after the event, organizers said the step-by-step scenario made the theoretical possibility seem very real for participants. In particular, it drove home the need for speedy, accurate information-sharing and strong coordination within and across governments and institutions.

In today's fast-paced world, information flows through unofficial channels much faster than through official ones, said Ron Klain, who was the United States' Ebola czar during the epidemic and served as the moderator for the simulation.

"Government officials need to be more in sync with that and adjust to that," Klain said. Officials who rely on once-a-day updates in outbreak situations need to understand the importance of releasing accurate information in real time, he said.
"I think the exercise clearly flagged that, and the ministers and others really focused on that," he said.

Participants in the simulation were shown hypothetical social media posts. One news story about a cruise ship passenger who worked in a lab researching the virus was seen by thousands of people within minutes of being posted on Instagram.

That part of the simulation was similar to what happened during the Ebola epidemic. A U.S. Coast Guard helicopter and plane were dispatched to a cruise ship off the coast of Mexico to obtain blood samples from a passenger on vacation. She had, 19 days earlier, been working in a lab at a Dallas hospital and possibly had come in contact with a sealed vial of blood belonging to Thomas Eric Duncan, a Liberian who died in October 2014, the first person to die of Ebola in the United States.

She had no symptoms, and blood tests later confirmed what doctors already knew: She did not have Ebola. (The only two people who caught Ebola in the United States were nurses caring for Duncan.)

The World Bank conducted its first pandemic simulation for finance ministers at its annual meeting last year. In January, the bank and the Bill and Melinda Gates Foundation hosted another simulation for chief executives of major companies at the World Economic Forum's annual meeting in Davos, Switzerland, in January. In July, they worked with German Chancellor Angela Merkel to conduct a pandemic simulation for health ministers at the G-20 governments meeting, which traditionally focus on finance and economics.

In the past, some countries have been reluctant to publicize outbreaks or delayed reporting them because of politics or worries about tourism and trade. In countries dependent on tourism, there is also political and economic pressure to take action that may turn out to be counterproductive for their economies, said Ryan Morhard, project lead for global health security at the World Economic Forum, which helped organize the World Bank simulation.

Imposing quarantines or closing borders may be seen as aggressive action, but those measures wouldn't necessarily be effective against a disease that spreads without symptoms, and could be "equally devastating to that economy," Morhard said.

Coordination also requires health ministries to work across the government with the finance and tourism sectors. "But that's not the way governments are usually set up any place in the world," Klain said. Most government systems "are really built in silos, nationally and internationally."

Although there is more awareness about the issue, experts agree that the world is not prepared for the next pandemic.

"We still are not ready for the big one," Klain said, noting that next year is the centennial of the great influenza pandemic of 1918 -1919 that killed at least 50 million people worldwide.

"We're frankly not ready for a medium-sized one. The threat is still out there."

Sooner than we expect? Later than I expected actually. I thought that the Ebola a few years ago had a real good chance of getting out of control and for awhile it did. It wouldn't take many cases of such a type to overwhelm the pitiful amount of isolation beds in the U.S. You could quickly go from 1st world to 3rd world level of medicine even without Obamacare.

Yeah, historically we are well overdue for a depopulation. Give the choice of Ebola, Smallpox or the Black Plague, I'll take the plague thank you. It seems mercifully the fastest.

It is not merciful but it can be faster. I would rather a pandemic then a nuclear war.

So when does a terrorist cell get infected fleas and spread them around the US? It seems like something like that should have already happened by now.

Considering that in the US there are around 400 cases a year anyway, not sure that this is anything to worry about....or should I run out and get some flea powder?

If the NorKs did try to hit the US with a bio attack, they'd be suffering from it themselves within a couple of days.

unfortunately a bio weapon that would kill off North Korea fits the plans of the Little Fat Boy perfectly - he'd want to see the results of his deadly handywork before NK would get blown away - seeing reports of the plague spread and the body count would probably kill the sucker with a glee overdose ...

a 12 Monkeys type bio weapon spread is eazily within NK's means ....

This is a very real nightmare scenario. Pneumonic plague, with its' onset and fatality ratios, and with airborne contagion potential, has a possibility of becoming very real.

And those semi trailers aren't just going to California.

Walking dead spreading death and disease. Some real doom there.

Most of the cases the USA has dealt with were NOT pneumonic, and the few that were, were NOT in highly URBAN areas!

Delaying more than ONE DAY to get treated makes it more likely than not that you will die.

An antibiotic resistant strain of Pneumonic Plague could circle this planet in just a matter of a day or two and be everywhere within a week.

Without a means to treat it we would have a complete global catastrophic disaster within a week, two at the most. Once the support systems that we rely on for our daily survival; water, sewer, power, fuel, food, etc., etc., started to fail it would be just one domino after another as things cascaded out of control. Inside a month it would be total anarchy. Within six months global devastation.

If you want to "prep" for this, go buy at least N95 or better MASKS, (without EXHALE valves) and antibiotics that treat plague!

Short of a "moon suit" and total isolation from friends, non-co-habiting family, neighbors, coworkers and all other people for ALL members of your family (including pets) until it all blows over and the dead are buried, you can just pray. Any family member NOT co-operating with the self-quarrantining voluntary isolation of the healthy, for survival, should not be allowed to re-enter the home, untill the epidemic has passed.

Control of food, GMOs, vaccines, reducing fertility, chem-trails and all the conspiracies you can count are slow and have limited effectiveness. Chemical agents are more effective but leave a trail to the perpetrators.

Biological was found to be the most effective method but lack of control has prevented its implementation. At some point evil will influence someone or group in power to disregard the risk or believe they are protected. Then you will see it used.

Perhaps NK will lead up to this.

I cannot cite specific sources for all of this but a book by Gary Kah, "En Route to Global Occupation", seemed to summarize the thinking on this issue in a fairly concise way. I am sure others can suggest more recent material.

As a major apocalypse event, a pandemic would be the easiest for many of us to survive. Those of us living in the country with a good supply of basic needs on hand could just hole up and wait.

Actually, a worst case, radical world depopulation epidemic event, WOULD COMPLETELY destroy any reasonable hope or chance for a "Global government" for at least the next 200-300 YEARS!

Because of the INDISCRIMINATE deaths, we might completely LOSE the best and brightest or even ALL the most prominate gifted, people in innumerable fields of human endeavor and could well be thrown back GLOBALLY to a pre-industrial age and society, just to survive at all, for many, many years!!

As a major apocalypse event, a pandemic would be the easiest for many of us to survive. Those of us living in the country with a good supply of basic needs on hand could just hole up and wait.

Theoretically, afterward, you'd then be able to restock your larder with salvage after the die-off that followed. Kind of like The Stand.

According to health minister Dr Ruth Aceng, their blood samples have been sent to Uganda Virus Research Institute for testing

Two health workers who treated a confirmed case of Marburg in Kapchorwa Hospital have developed Marburg – like symptoms.

The virus was confirmed a few days ago in Uganda.

According to health minister Dr Ruth Aceng, their blood samples have been sent to Uganda Virus Research Institute for testing.

“In the interim, these health workers have been isolated and are being monitored and counseled by our experts,” says Aceng, in a press release.

By yesterday a total of 155 contacts had been line-listed in Kapchorwa and Kween districts; 109 in Kween district and 46 contacts in Kapchorwa district.

In Kween district, 66 of the contacts have passed the mandatory 21 days of follow up from the time of contact with the first probable case and the remaining 43 are still under contact follow up by the teams on the ground.

In Kapchorwa district, 22 of contacts have also passed the mandatory 21 days of follow up from the time of contact with the first probable case and the remaining 24 are still under contact follow up by the teams on the ground.

“We have assembled a team of seven contact tracers, trained them and provided them with support to conduct adequate contact tracing. Our multi- stakeholder teams of experts will remain on ground in Kapchorwa and Kween district until the outbreak is fully contained,” Aceng noted.

Earlier, the minister said there is no drug to cure the virus and advised the public to report suspected cases.

She advised the public to avoid contact with patients suffering from Marburg, since the virus can be contracted through body fluids.

Symptoms may include

Nausea and vomiting

Diarrhea (may be bloody)

Red eyes

Raised rash

Chest pain and cough

Sore throat

Stomach pain

Severe weight loss

You are exactly right.
 
During the mid-1990s, FEMA war-gamed various scenarios of WMD.

As mentioned above, bio attacks were considered less likely because of the inability to control the resulting pandemic.

Bio-Chem scenarios included terrorists introducing air-disbursed bio-chem agents into the ventilation systems of major city office and residential high-rise buildings.

Another looked at the ability of a highly contagious bio agent to spread/propagate, when carried by a few unsuspecting airline travelers making connections through busy international airports, who show little to no symptoms, personally, while spreading disease.
