I was actually joking around with my roommates last night and ended up asking this question to them... We came up with a plan to buy Philadelphia and make it awesome.

TB2K weekend getaway (not so much a weekend getaway as a multi-year getaway to secure underground locations).

I'd fund Armadillo Aerospace, JP Aerospace, Bigelow Aerospace and probably the Mars Society until they all could outfit me with a robust way of immigrating off of this planet and setting myself up someplace else in the solar system, along with whatever family and friends that wanted to join me.

Let's see...the 2006 estimated GDP was $13.13 trillion (https://www.cia.gov/library/publications/the-world-factbook/print/us.html) and it's estimated that we spend about 15.2% of that on health care--or roughly $2 trillion per year, if I'm doing my math right.
It'll be a nice few months, anyway.
If you're going to go the island route you might want to look into picking up one of the old US aircraft carriers that are waiting to be scrapped and build that into an 'island'.
