CDC weighs loosening guidelines for some exposed to virus

WASHINGTON (AP) — The Centers for Disease Control and Prevention is considering changing its guidelines for self-isolation to make it easier for those who have been exposed to someone with the coronavirus to return to work if they are asymptomatic.

The public health agency, in conjunction with the White House coronavirus task force, is considering an announcement as soon as Wednesday, Vice President Mike Pence said on Tuesday.

Under the proposed guidance, people who are exposed to someone infected would be allowed back on the job if they are asymptomatic, test their temperature twice a day and wear a face mask, said a person familiar with the proposal under consideration. The person described the proposal on the condition of anonymity because the draft had not been finalized.

The new policy is aimed in particular at workers in critical jobs. But it also comes as the Trump administration is eyeing what it calls a “stabilization” in infection rates and looks toward rolling back some of the restrictive social-distancing guidelines and restarting the nation's stalled economy.
The proposed guidance would follow recommendations made by the CDC that eased self-isolation requirements for front-line medical workers who were exposed to the virus. Under CDC guidance, medical workers who have been exposed to the virus without protective equipment but who have no symptoms can return to work with a mask and temperature checks after 14 days.

Pence on Tuesday said the White House is focusing on the “point of need” for the current situation but is also operating on another track to consider future recommendations for the public.
“Some of the best minds here at the White House are beginning to think about what recommendations will look like that we give to businesses, that we give to states, but it will all, I promise you, be informed on putting the health and well-being of the American people first,” Pence said.

For most people, the new coronavirus causes mild or moderate symptoms, such as fever and cough that clear up in two to three weeks. For some, especially older adults and people with existing health problems, it can cause more severe illness, including pneumonia, and death.
In fashioning the recommendations, the administration appeared to be trying to balance political concerns about wanting to preserve as much normalcy as possible with public health concerns that some infections are being spread by people who seem to be healthy.

Ozone will kill living organisms. Including humans and animals if you stay in the room where being used and will impact plant life. Make certain that all animals, including humans, and plants are removed before using a ozone generator in a room. Ozone will also destroy odor. Ozone will dissipate once the ozone generator is turned off. Also open up a window once the ozone generator is turned off. Many ozone generators come with a time switch.

Use carefully and read the instruction book before using.

Does Estrogen Protect Women From Coronavirus?

New information about how estrogen may be protecting women from serious infection!

The new coronavirus has only been around for a few months, so research is just beginning and there is still so much we don’t know. A new Chinese study looked at the receptor used by the virus to get inside our cells and gives us a glimpse into who is more susceptible and why. Please be aware that this information is not yet peer reviewed so it is only preliminary and the facts could change.

When looking at the current COVID-19 pandemic, we see that children they tend to have mild disease. The risk of serious disease increases with age. Women are less likely to have severe disease than men. People with pre-existing diseases, such as diabetes, heart or lung disease or weakened immune systems are also at higher risk. The highest risk category are elderly men with multiple pre-existing conditions. This new research helps explain why.

How does the virus get into our cells?
We know that the virus uses a receptor called ACE2 as a doorway to get into our cells. It would make sense that the more of these receptors (doorways) you have, the greater your risk for disease.

In fact, the study found the exact opposite! People with the most ACE2 receptors had the least risk for disease. We don’t understand why people with more receptors are more protected, so we’ll have to keep learning.

Why children and women have lower risk
I attached a link to the study at the bottom, but it is not easy reading so here is a summary of the findings:

AGE: Young people have more ACE2 receptors (lower risk) and the number of receptors goes down with age (increased risk). This may explain why children are at such low risk of severe disease – they are protected by having lots of these ACE2 receptors.

SEX: Women have more ACE2 receptors than men, and in fact we see women tend to have less severe disease than men.

HORMONES: The researchers surmised that estrogen may be protecting women. They looked at women on estrogen treatment and found that women on estrogen had more ACE2 receptors (associated with less risk). They also looked at men on testosterone therapy and found a slight increase in receptors (although not nearly as much as with estrogen). When they looked at male to female transgender people (on estrogen replacement and testosterone blocking medicine) they found significantly higher ACE2 receptors.

DIABETES: People with type 2 diabetes have significantly fewer ACE2 receptors, and more risk for disease. The study did not include information about other diseases but perhaps other disease conditions also reduce receptors – more research will be needed here.

MEDICATION: ACE inhibitor medications are often used for high blood pressure. These medicines block the effect of the protein produced when the ACE2 receptor is activated We don’t yet know what effect the medications could have (if any) on disease risk. It is possible that ACE inhibitors could reduce risk, but we just don’t know. It is not recommended to change medications at this time since we just don’t have enough information yet.

How does this information help?
We can’t change our age or our sex. I am not recommending that anyone run out and start estrogen or testosterone therapy to avoid coronavirus infection.

If you have type 2 diabetes, you can certainly take charge of your health and improve your lifestyle habits. It’s not too late to start today! We don’t know yet how blood sugar control will affect the number of these receptors, but we know that poor blood sugar control weakens your immune system, and better blood sugar control allows your immune system to be stronger.

We don’t really understand why having more ACE2 receptors reduces the risk of severe disease, but perhaps this will help us to learn how to prevent or treat coronavirus infections in the future.

US Military transferred 18 coronavirus patients after an emergency with oxygen at local NYC hospital

A New York City hospital had issues with its oxygen supply Tuesday night causing the urgent transfer of multiple patients to be cared for elsewhere, Gen. Terrence O’Shaughessy told CNN.

“They needed to transfer those patients immediately as quick as possible because their lives were dependent on it,” said O’Shaugnessy, the commander of US Northern Command overseeing the military response.

A team of 37 military doctors, nurses and technicians in full personal protective equipment moved 18 patients in an ambulance to different facilities for urgent care.

Ten patients were moved to the USNS Comfort, five to Sinai, two to Bellevue and one to North Bronx, the general said.
O’Shaughessy said the military is there to be used in such an emergency at area hospitals.

“It was challenging, they were all in various significant states of acute medical needs,” he said.

“When they got to (the) Comfort, several of them were crashing so they had to provide that immediate medical assistance ... As they were pulling up to the Comfort they were crashing so the doctors from the Comfort had to respond in the ambulance and then get them stabilized and then bring them into the Comfort.”​

A source with knowledge of the operations at multiple hospitals said the New York City area hospital had “major problems with excess ice accumulation on their overburdened medical oxygen systems.”

I think it has more to do with production lines than amounts - if certain trucks are supposed to pick up from certain farms (i.e. onion farm for restaurants) and then bring that product to places that process the produce - chopped onions, onion rings, etc... And the processing plants are either shut down or their freezers are full - the production line stops. So, we can go to the store and see that there's not enough milk - but that production line is retail, so milk is being dumped because the other production line (restaurant) has been stopped. And, I agree - at some point, we are not going to have enough food.
That makes sense. We are going to have a food shortage worsened by a distribution issue.

In the span of two hours Wednesday, the state of Kansas reported its biggest single-day increase of both confirmed cases and deaths from COVID-19, a legislative committee voted to revoke Gov. Laura Kelly’s executive order barring religious services from holding gatherings over 10 people, and Kelly lambasted Republican state officials for playing politics during a deadly pandemic.



The move to rescind Kelly’s order limiting the capacity of in-person religious gatherings — which took effect Wednesday morning — may have also invalidated the statewide ban on mass gatherings entirely.


Kansas House and Senate leaders, who form a 7-person group called the Legislative Coordinating Council, voted 5-2 along party lines to rescind the order in a move Kelly said rendered her speechless and created statewide confusion.


“There are real life consequences to the partisan games Republican leaders played today and I simply cannot stand for it,” Kelly said in her daily COVID-19 press briefing. “There is no room or excuse for these petty political distractions.”


Now, instead of focusing all of the state’s energy on combating the deadly respiratory virus, Kelly said her staff now has to divert attention to understanding the legal fallout from the LCC’s action. The revocation of the executive order could potentially invalidate all of Kelly’s previous actions — namely statewide stay-at-home orders and limits on mass gatherings.


Kelly’s chief counsel is reviewing all options to address the LCC action, she said, and did not rule out a legal challenge.


“Nothing is off the table,” she said. “We will do what we need to do to make sure what is in place is in the best interest of Kansans.”


The striking comments, by far Kelly’s most aggressive against her Republican colleagues since she took office last year, came shortly after the state Department of Health and Environment announced that overnight, Kansas confirmed 146 new cases of the virus – raising the cumulative total from 900 to 1,046 — an increase of 16.2% and the biggest total daily case increase since the outbreak began.


The state also confirmed 11 more COVID-19-related deaths — also the most in one day since the outbreak started — bringing the total to 38. A single-day increase of 40%, Kelly said the somber numbers put the state “on alert” and chastised Republicans for taking action that could further spread the virus.


The 1,046 confirmed cases are out of 10,183 total tests, meaning 10.3% of tested Kansans have the virus. A week prior, on April 1, only 8.2% of all tests returned positive.


Those numbers should be taken with the caveat that COVID-19 tests potentially have a high false-negative testing rate, according to recent research, as well as the fact that widespread testing is still not available in Kansas or the rest of the country.


KDHE last week began releasing more comprehensive data in its daily updates, which now include the testing rate in each of Kansas’ 105 counties. In Douglas County, 910 people have been tested either by the state or by private labs, and 38 cases have been confirmed positive.


The testing rate, the department said, equates to 7.44 tests per 1,000 county residents. That’s the third-highest rate among all Kansas counties.


KDHE is also tracking hospitalization rates for cases where such information is available. The department said 248 of the 771 positive COVID-19 cases that are being tracked have resulted in hospitalization thus far — a 32.2% rate for applicable cases.


Department Secretary Lee Norman, who appears with Kelly at briefings on Mondays, Wednesdays and Fridays, again reiterated that 3 of the 12 confirmed clusters of COVID-19 have been linked back to church gatherings.


And while Norman said he’s not a politician, he was disappointed over efforts to rescind gathering guidelines — especially since Kansas will soon reach the peak of the virus outbreak.


“It is a quick and devastating impact that gathering can have on spreading this virus,” he said.


“If we go lax at this critical juncture, lots of people will get sick very quickly. As a state health officer, I was disappointed by the actions today.”

The coronavirus seems to be getting worse, the government is requesting Americans return home and we have a home in Alaska. Thus why we are contemplating making the 4000 miles journey north. Firstly we need to drive through 4 military inspections, one state line and an international border with the United States of America.

I searched forever to find it, again, but I couldn't ( if someone knows the study I'm talking about and has a link, it would be appreciated ), I read that the dosage was supposed to be something like 2mg for every pound of bodyweight. I'm 250lbs so my daily dosage would be 500mg...

NEW YORK (REUTERS) - Some coronavirus patients who would have been admitted into the emergency department at a New York hospital are being sent home with an oxygen-monitoring device as the city's medical system struggles to reserve resources for only the sickest people.
The new programme at NewYork-Presbyterian Hospital is an example of how doctors are adapting and loosening normal protocols to ease the strain on emergency rooms and intensive care units in New York state, the epicentre of the coronavirus pandemic in the United States.
Since last week, more than 200 people with confirmed or suspected Covid-19, the respiratory illness caused by the virus, have been sent home with a pulse oximetre to track their oxygen levels. A doctor or nurse practitioner follows up with them via video conference.
"Some of these patients might have been on the borderline of admission," Dr Rahul Sharma, who is overseeing the programme as the chief of emergency medicine at Presbyterian's Weill Cornell Medical Centre, said in an interview.
An oximeter is a small electronic device that clips onto a fingertip to indirectly measure the oxygen saturation of a patient's blood. In severe Covid-19 cases, the virus can block up the lungs, hindering their ability to pass oxygen from the air into the bloodstream. While most who contract the virus recover, it has killed at least 4,900 people in the city, according to a Reuters tally.
Some of the NewYork-Presbyterian patients are also being sent home with a 14 kg portable oxygen concentrating machine which sends oxygen-rich air through a nasal cannula, a two-pronged tube inserted into the nostrils.


The patient is asked to log their oximetre readings to share with a doctor or nurse practitioner at 12-hour and 24-hour consultations. The patient may be re-admitted to the hospital if they take a turn for the worse.

No patient in the programme is known to have died at home as of Wednesday, but about 15 per cent were asked to return to hospital to be admitted for inpatient care, a hospital spokesman said.
At least two other hospital systems outside of New York unveiled similar new programmes using oximetres in late March.

Doctors at other hospitals said Presbyterian's approach could have benefits for patients with a disease about which they still have much to learn.
"There's no guarantees in either direction, there are risks to hospitalisation and there are risks with sending people home," Dr Laura Burke, an assistant professor of emergency medicine at Harvard Medical School, said in an interview.
Doctors have noticed there can be a worrisome lag in symptoms among some Covid-19 patients, with oxygen saturation dropping for some time before a patient is aware of breathing problems.
For patients that turn up at an emergency department with other Covid-19 symptoms, giving a pulse oximetre to use at home could be one way to "err on the side of safety," Burke said.
