RESQDOC made the comment one day (we had all been into his Balvenie Double Distilled) that if he'd had it to do over again he'd have gone the PA route. Best job in medicine. And if yer like him or me, who read FURIOUSLY and WIDELY your knowledge base can be as good or better than the doc in the next treatment room.

It can't be contained. BUT, individual families could significantly limit their exposure to the contagion and lessen their chances and their families chances of getting the bug IF the government would get it's head out of it's A$$ and push anal retentive control freak supervisors to let government, civilian and military who can work from home to work from home. Cyber schooling kids would also be a big plus on the limiting exposure to the bug. IMO the teachers Union would fight it tooth and nail.

Many teachers are using google classroom anyway. The biggest concern would be verifying that it is actually the student doing the work.

OH, My Apologies for appearing to troll. I honestly thought I was responding to someone who had requoted y'all and was trolling with the repost.

Been a rather weird week.

(You oughta know by now, we been in this for the last 20 years) that some days I be dumb as a box of hammers. Also I found out today that my peripheral awareness, and situational awareness, well, SAOAFR covers today for me.

SORRY!!

The RC Bishop of Hong Kong has closed his churches and told everybody to stay home. Which leads to a story that I might have told in olden times.

Grandma was dying very uneasy in her mind. She wanted her minister to pray over her in her last hours. So the family called the minister but no go, he was not available,

So the nurse says call the priest, he always comes.

Well, the family didn't think Grandma was really into priests so they tried another minister with no luck.

And the nurse says, call the priest, he always comes.

By this time Grandma is really uneasy and maybe she might not notice. So they called the priest.

And he showed up with three other people.

And he says a prayer over Grandma which everybody including Grandma thought was pretty good.

Then the three companions step forward, announce that they are going to sing one of those rock solid Protestant hymns and invited the family to participate which they did and Grandma seemed to appreciate that too.

Then the priest stepped forward and read from the Bible (II Timothy?) and Grandma relaxed, everything was OK now.

The priest always comes. Well, you are the Bishop and you have this plague that is wiping out people right and left because it is incredibly contagious. Do you stand and watch your diocesan clergy get wiped out? But, approaching the dying while wearing a space suit does not seem to add to the ambiance usually associated with succoring the dying. .

This thing hits like it has other places, gonna be some gut-wrenching decisions.

It doesn't. i don't have much in the way of experience with the vet side of things, but the principle should be the same. Having the equipment to really do the job--sterile--would be a challenge. Remember, you are intubating right into the nasopharynx where there is a high viral load. Tickle the wrong part and you will be on the wrong end of infected vomit.

If the need arose I would imagine everything would be tried.

Most people that would be hospitalized wouldn't need ventilation or ECMO. a high flow mask would probaly be adequate in most patients. The autopsy I read the person was desatting but declined to be intubated because of claustrophobia. Even on high flow O2 his O2sats dropped into the 60s. They did intubate him but he coded shortly thereafter and passed.

Wish I had more valuable input, but this is out of my area.

Ok. Thank you for the reply.
The sterility issues are what I thought of first, right behind the mechanics.
We sterilize quite rigidly, but not nearly what I’d imagine is used humans, and yes, the comment about vomit IS a thing I didn’t think of.
I guess, if I had a loved one who needed help bad enough I’d at least entertain the thought of using vet facilities if nothing else was available.
Hmmmm... good food for thought there, and great for a brain storming exercise.

(On a side note?... human bodily juices and secretions gross me out wayyyy more than animal issues any day of the week. Humans are kind of gross, in my estimation. At least animals are honest ...***shrugs***)

This too shall pass.
Actually I like them and have gotten better care from them than actual doctors...

This has been my experience, as well, with three different PA’s in three different locations. They don’t seem to be in such a huge rush, and take time to listen and try to actually find out what is going on. Their diagnoses are at least as accurate if not more so. I was glad to find out my daughter would be seeing a PA at the clinic I‘ve been taking her to.

Regarding sterility . . .

Here's a journal article that says you can use an InstantPot as an autoclave:

That is cool!

Ahh, just a comment, that the last intubation I did in a clinical setting the Anesthesia guy who was passing thru pointed out that intubation is NOT a sterile procedure BECAUSE the mouth and nose are TEEMING with nasties (human bite is the most contaminated bite out there WELL ahead of cats which IIRC were #2)

If I'm the Bishop the priests get PPE's and training to help prevent them getting the disease and spreading the disease.

Congregations and mourners are told to stay home and participate in Church services and funerals via live stream on the internet. I remember some churches broadcasting services on the radio.

Unless the government intervenes this is not going to happen. They can beg donations but can't pass the collection plate on radio or internet.

You are absolutely right...I was thinking keeping the person intubating clean. I should have clarified that!

So they found 20 new cases in South Korea on the 19th, 53 on the 20th, 100-odd on the 21st, and now 142 new cases today? Yeah, they've got a problem.

If we don't have VERY similar numbers the 2nd and 3rd week in March we will have dodged a HUGE bullet.

NOT holding my breath though.

OH HELL!! Those look exponential (factor of 2) on a less than 2 day cycle....(or Gawd forbid a factor of 3)

No. This was the second week in January. It was an urgent care doctor, not my GP. She prescribed me some antibiotics because she thought I may be getting a sinus infection and sent me home.

Edited to add - This wasn't really being talked about in the news yet. There were no quarantines. I didn't even know the Chinese kid had been sick until about a week after I had already been to urgent care and then I started putting it together in my head.
Might not hurt to bring it up. Or, maybe they'll put you in isolation. Who knows?

Helen dropped this on the CSue thread. I brought it here.

Curiouser and Curiouser ...

Go to Twitter.

Use the search function.

Search term is monkeypox with no hashtag.

A guy uses that for his handle, so you'll have to wade through his stuff to find articles from African news and health organizations.

Monkeypox showed up over 200 times in 2019.

Monkeypox showed up more than 200 times in January 2020 already.

Symptoms look like any other flu virus at first.

Looks like it, yeah, but I think it's more likely that some of these people have had the bug for a while, it's just that testing has stepped up around the church-related super-spreader incident. We've got no idea how many of the recently detected cases are symptomatic, how many were just tested because they were known contacts, etc. etc. Considering the high percentage of mild cases and the potential length of both incubation and shedding periods, we could have more than one generation being detected at once.

Where to purchase the Chaga Mushroom????

Thanks,

Dragon Herbs

I haven't used Dragon Herbs, it may be less expensive there so check them out too. I was going to buy some powder form from Amazon. I buy powder so I can control the dosage, I like to start out really small and make sure there's no problem before I take the recommended or above-recommendation dose. I know I'm repeating myself but if anyone is on a prescription medication or has an underlying condition, you have to check there is no drug interaction or contraindication when taking an herbal remedy. You usually can google it.

US changes the way it counts coronavirus cases
From CNN's Ben Tinker

4442fa82-d6ab-4b06-aab7-75fdfbd8cbe4.jpg

The US Centers for Disease Control and Prevention will now track confirmed cases of novel coronavirus in the United States in two separate and distinct groups: those repatriated by the US Department of State and those identified by the US public health network.

What this means: By these new metrics, there are now 21 confirmed cases among repatriated Americans and 13 confirmed cases among Americans first identified on US soil, said Dr. Nancy Messonnier, director of the CDC’s National Center for Immunization and Respiratory Disease.

Eighteen cases have been confirmed among passengers from the Diamond Princess cruise ship now back in the United States. Three cases have been confirmed among passengers from the Wuhan repatriation flights now back in the United States.

Up until yesterday, the CDC was reporting 15 confirmed cases in the United States. The reason that number has dropped to 13 is because two cases have moved into the case count of repatriated individuals.
The change in the counting system starts today.
