China Applies Stricter Criteria for Diagnosing Covid-19 Cases

China’s top health authority changed its diagnostic guidelines for Covid-19 once again, applying stricter, unified criteria nationwide rather than a different, broader standard only for Hubei province as in the previous version.

The sixth national treatment and diagnostic plan for the deadly coronavirus issued Wednesday by the National Health Commission reverses a change in the fifth edition released last week. Under that plan, cases in the epidemic’s epicenter Hubei could be diagnosed clinically, such as via chest X-rays. Now all confirmed cases must be determined using laboratory tests known as nucleic acid tests (NATs).

The new, stricter diagnosing criteria are likely to result in further declines in the rate of reported new cases. The previous change that allowed chest X-rays to be used for diagnosis alongside standard lab tests resulted in a one-day jump in Hubei of nearly 15,000 new confirmed cases on Feb. 13. About 90% of them came from a clinical diagnosis.

As of the end of Tuesday, there were 74,279 confirmed cases of Covid-19 in China and 5,248 suspected cases, according to data from the National Health Commission.

Previously doctors raised concerns about false negatives from laboratory testing that sometimes failed to diagnose infected people. The new guidelines stress the accuracy of the lab test.

To address concerns about false negatives, the new guidelines emphasize the use of sound sampling procedures to safeguard the accuracy of the findings and suggest using multiple NATs alongside a new, rapid antibody screening test kit developed by Guangzhou Medical University under the guidance of Zhong Nanshan, a respiratory expert who discovered the SARS coronavirus in 2003.

The new test kit can detect an immunoglobulin produced only after a human body has contact with the Covid-19 virus, Zhong said Tuesday at a press conference in Guangdong. The use of the antibody screening test can be mutually verified with the NAT results and can help to identify suspected cases while increasing diagnostic accuracy, Zhong said.

The new test kit needs only one drop of blood and can produce results onsite within 15 minutes, according to a developer of the test kits. But experts said NATs are still considered the “gold standard,” while the sensitivity of the antibody test needs further clinical verification.


It takes time for the human body to produce antibodies, which are closely related to a patient’s own immune system. Consequently, antibody testing can fail to detect infected people, said Yao Tiancheng, chairman of Shenzhen Tisenc Medical Devices Co., one of the developers of the new test kits. The antibody testing and NATs cannot replace each other, he said.

To lower the risks of missing any infected person, the new guidelines also tighten the criteria for lifting quarantine for recovered patients. Under previous guidelines, people with two negative NATs tests could be released from quarantine. Now, in addition to that requirement, patients must resume normal temperature readings for at least three days and show significant improvement of respiratory symptoms and pulmonary imaging before their quarantine can be removed.

The health authority previously said there is no proven effective treatment for the coronavirus. The new guidelines remove that statement and add the antiviral drugs chloroquine phosphate and Arbidol to the treatment plan. But it is not recommended to use more than three antiviral drugs at the same time.

Chloroquine phosphate, a drug usually used to treat malaria, has been evaluated in hospitals and proven effective in treatment of the coronavirus, said Sun Yanrong, deputy head of the China National Center for Biotechnology Development under the Ministry of Science and Technology, at a press conference Monday.

The guidelines recommend the use of blood plasma from recovered people on patients in severe and critical condition. However, due to a limited plasma supply, it is difficult to apply on a large scale.

The new guidelines also for the first time raise the possibility of transmission through aerosol — tiny particle droplets suspended in the air ― in closed environments for extended periods of time.

Unfortunately I deleted them after I watched them so have to find the links again. Yep, I watched it live and it drove me insane, also the sound was so distorted I couldn't understand a word the Chinese gentlemen said for the second half of the show (and growing up in California I can usually understand them) as well as the voices talking over the program.

Plus I lost not one but THREE feeds before I managed to get one that didn't go down or that actually worked for video.

I told Nightwolf the whole thing made me even more determined to watch and it reminded me back in the days when I was on TimeMonk Radio and we would interview Cliff High, the feeds would constantly get cut off and made some of the interviews unintelligible.

I figure when "they" are bothering to do that, there is something important being said, even if I'm not always sure what it is they are trying to censor - though in this case, it is pretty obvious.

Did you notice the Chinese gentleman say the count was 4-5 million instead of the 75,000 they said it was? Anything worth their time to censor is worth my time to listen carefully. Even if that was very hard to do. It is up to me to figure out what is true, not up to them to hide the real facts and tell me what the truth is. If you think I'm hot, the people of China are hotter. Civil War soon. Bannon is doing some amazing work IMHO. History in real time.

The cruise industry is “focused on protecting the health and safety of passengers and crew” amid the coronavirus outbreak, according to the industry’s largest trade group.

The enhanced screening measures, the first of which were adopted within hours of the WHO declaring a global health emergency, do appear to be working and have enabled operations to continue as usual in many parts of the world,” said Bari Goin-Blaugrund, senior director of strategic communications of Cruise Lines International Association, or CLIA.
Goin-Blaugrund also said that within the global CLIA fleet of 272 cruise ships, only one has experienced any cases of coronavirus on board.


Passengers aboard that ship, the Diamond Princess in Japan, began disembarking Wednesday after a 14-day quarantine. A total of 624 cases of the virus are linked to the stricken ship.

Analysts say the coronavirus could take a financial toll on the industry, especially in China. If operators cancel cruises through March, JP Morgan predicts revenue could fall as much at 15%.

Shares for two of the biggest cruise operators — Royal Caribbean and Carnival — are down more than 16% and 14% this year, respectively.

No US coronavirus cases have been caught by airport temperature checks

Since mid-January, the US Centers for Disease Control and Prevention has been employing temperature checks at selected airports, at first on people flying in from Wuhan, China, the epicenter of the coronavirus outbreak, and now on all passengers who've been anywhere in mainland China in the past 14 days.

While the CDC has screened more than 30,000 passengers in the past month, not a single US coronavirus case has been caught by airport temperature checks, according to a CNN investigation.

CNN reviewed statements by local, state and federal health officials, and emailed and spoke with officials, to determine how many people eventually diagnosed with the novel coronavirus went through airport temperature checks.

By the numbers: Ten passengers flew into US airports on commercial airliners and were later diagnosed with coronavirus.
Four of those 10 passengers went through temperature checks and were found not to have a fever or any other symptoms of the virus. Two of those passengers flew into Los Angeles International Airport and two flew into San Francisco International Airport. Previously, a CDC spokesperson had told CNN that two of the passengers were symptomatic during flight, but the spokesperson later said they were asymptomatic while flying.

Another four out of the 10 passengers were not screened because they flew into airports that did not have screening at the time. Those cases are in Illinois, Washington state, Massachusetts and California.

It's unclear whether the final two passengers went through temperature checks because health officials at the CDC and in Wisconsin and Arizona refused to say when asked by CNN.

How the screening works: The temperature checks are part of an overall screening effort that includes giving passengers cards telling them to watch out for fever, cough and trouble breathing, all symptoms of the novel coronavirus disease, also called COVID-19.

While airport temperature checks haven't caught any coronavirus cases, the information cards have helped catch US cases of the virus at least twice – once in a passenger who flew into San Francisco, and once in a passenger who flew into Los Angeles.

I can't find the link, it is a very confusing forum to navigate. I didn't want to make the impression it was a Corona virus, but I believe they were odd pneumonia/flu type cases.
I didn't misunderstand you, no worries. I was extrapolating from what you said that it might have been this virus as it first emerged from the bats and snakes and whatever else it emerged from and it took 5 or so months to mutate into this virulent version. I'm not a dr or any type of medical person so that's just conjecture from reading about how these diseases supposedly work.

Yes I did hear at least part of that, but I wasn't sure that was exactly what he said - it wasn't his fault, the sound kept distorting and I lost the feed that had subtitles (I use them when I can because I am somewhat hard of hearing anyway).

But I feel exactly the same way you do, and while I don't always agree with Bannon on everything, I told Nightwolf he seems to be doing the best job he can on this one.

He also didn't contradict the doctor who was explaining that in his view, such and such paper convinced him that it probably wasn't a bio-weapon which is fine - I don't know enough to be sure either way (and Steve probably doesn't know either) but that still doesn't mean that it didn't escape from that lab.

As Nightwolf told me it only matters in terms of trying to treat it, as a practical matter it is now out there and "someone" in China and the rest of the world knows something about it most of us don't.

You simply don't shut down a country like China over a flu outbreak (for weeks) and what is happening in Persia's main religious center/city is scary as heck (they probably know something too or their government and scientists do).

Coronavirus patients can have similar "viral load" whether or not they show symptoms

People with the novel coronavirus may have the same viral load — or amount of virus in the body — whether or not they are showing symptoms of illness, according to researchers in China.

The letter from the researchers, which was published in The New England Journal of Medicine, details how the researchers monitored viral loads of the novel coronavirus in samples taken from the nose and throat of 18 patients: nine men and nine women ranging in age from 26 to 76 in Zhuhai, in China’s Guangdong province.

For those samples, a total of 72 nasal swabs and 72 throat swabs were collected and analyzed.

The analysis revealed that higher viral loads were detected soon after a person was showing symptoms, with higher viral loads found in the nose than in the throat. Among the patients, only one was asymptomatic.

Yet overall, “the viral load that was detected in the asymptomatic patient was similar to that in the symptomatic patients, which suggests the transmission potential of asymptomatic or minimally symptomatic patients,” the researchers wrote in the letter.
“These findings are in concordance with reports that transmission may occur early in the course of infection and suggest that case detection and isolation may require strategies different from those required for the control of SARS-CoV,” the researchers wrote, referring to severe acute respiratory syndrome, known as SARS. “[W]e need better data to determine transmission dynamics and inform our screening practices."

It also turned out that, when it comes to viral shedding, the novel coronavirus seemed to resemble influenza more than SARS, according to the letter.

How many affected: Globally, the World Health Organization has reported that there have been more than 75,000 laboratory or clinically confirmed cases of novel coronavirus and more than 2,000 deaths.

Of course, 2 people didn't wake up in the morning totally fine and then die of COVID later in the day. Which they just happened to test for after they were dead. So who knows what's going on there, how many patients they have, and how many suspected cases. You don't lock down a city because 2 people just happen to die that day....

If they have to lock down a whole city of Persians, this is not looking at a bio-engineered weapon aimed at Asian markers. So this may be the scariest thing I've read yet. (or does Qom have a chinatown type area?)

For those that believe in spiritual gifts, the word I've been getting is "bloom." The interpretation of that is that I think we are going to see something akin to an algae bloom with this- sudden, explosive growth. We've already seen that in Wuhan. Hopefully it won't be as bad here or in other areas. YMMV.

Is it possible that all the Diamond Princess quarantine did was infect more people with Covid-19 and that the aftermath of the poorly-executed quarantine will spread the virus across the globe?
The only folks who are remaining in quarantine are those who were sharing a cabin with infected people and each day a few hundred more people will be released willy-nilly into Japan’s public transportation system to make their way back to 50 different countries – most likely on public transportation

Not necessarily. It appears that most non-asian ethnicity has a milder case and, with the amount of influenza and focus on that problem, most would attribute it to the flu and not coronavirus.

And with extended incubations, it may well be still too early.
Good points. But this stuff seems to be very contagious. So if there is something to note, it should affect a whole bunch of people, all at about the same time. And that would be noticed.

An unknown number of evacuees connected to the coronavirus outbreak have been transported from Joint Base San Antonio-Lackland to a state hospital in San Antonio.

Chris Van Deusen, a spokesman for the Texas Department of State Health Services, said that patients were transported to the Texas Center for Infectious Disease on Tuesday and Wednesday. He declined to specify the number of patients being treated there.

The 75-bed hospital typically treats only tuberculosis patients, but state officials have reserved 22 beds in a separate wing for people suspected of having coronavirus, as well as infected patients who are experiencing mild symptoms.

Patients who need a higher level of medical attention would be transferred to an acute care hospital in the area, Van Deusen said.

Reading this thread and pondering the current coronavirus situation remonded me of a 1972 movie featuring George C. Scott, titled "Rage". The movie's story line was about a father (Scott) and his son who are exposed to an Army nerve agent while on a camping trip. Both father and son receive lethal doses of the nerve agent, though the son succumbs first. Scott does his level best to exact revenge on everyone who was involved in the nerve agent exposure and its cover-up.

The movie illustrates phenomenon we are widely seeing with the coronavirus situation. There are coverups, government pressure on civilian doctors to toe the party line and the usual government mendacities we've all - sadly - become accustomed to.

Worth a watch if you can find it and - seen through the right prisms - will remind you of what we are experiencing with Corona.

There were 2 cruise ships affected. One had quite a few with coronavirus, the other docked in Cambodia, and a passenger from that particular ship was discovered sick in Malaysia. God only knows if anyone else she was onboard with was infected, not to mention on the plane she took to Malaysia.

My personal reaction when it finally sunk in that the Japanese had just released people on that ship even though a bunch of people had continued to test positive - I mean released them to "make their own way home" using cabs, airports, etc was to say out loud:

"That's not the end of a quarantine, that's the start of a biological timebomb"

There were 2 cruise ships affected. One had quite a few with coronavirus, the other docked in Cambodia, and a passenger from that particular ship was discovered sick in Malaysia. God only knows if anyone else she was onboard with was infected, not to mention on the plane she took to Malaysia.
