I ordered plugs last year of Reishi and Shiitake and did the innoculations exactly as directed. I still have yet to see any life from them. I did save half the crop and re introduced them to the logs this spring so we'll see if I have any luck. I also added some new logs to see if that was the problem. I bought them from Companion Plants and they are a good company so I may call them and get some advice. Their link is http://www.companionplants.com
You may have to call to order a catalog. They alos have pre-innoculated logs for sale. That might be the way to go.
