    Coming really... ACO's to strictly aggressively ration health care

    I operate a medical practice (independent physical therapy clinics specializing in musculoskeletal pain problems), so I stay very up to date re emrging health care regulations. Widely reported throughout various states local newpapers is confirmation that Medicare (and several private insurance companies) has launched a program called ACO (accountable care organizations) whereby all docs will be employed by their local hospitals, hospitals organized into larger corporate medical provider corporations, and all health care will be provided by that organization (private practices mostly excluded). The hospital corps will be paid a flat rate per person per year to provide all health care. Docs will be "encouraged" to keep health services minimized to reduce utilization of costly care (MRI and other costly imaging, minimize referral to costly specialists especially if they may be surgeons, minimize referrals to phsical therapy, anything that uses hospital resources). The goal is to reduce use of health care resources that provide care, so that hospital organization gets paid but does not expend those dollars on so much care, allowing them to keep the resulting surplus.

    The goal is laudable, ie, reduce health care costs. And it should reduce needless tests and procedures. But how far will they go to minimize costs so they can keep the surplus as profits? Docs will get bonuses or penalties based on how they do at rationing care. This is very similar to the HMO concept of the 1980-90s except the proftis stay with the hospital rather than the inusrance co. How far will they go?? What care will YOU be denied?? Sarah Palin death panels inching toward reality?

    Private insurers are on a similar path. Many insurers require patients needing PT to undergo review before care, will be allowed a few treatment sessions (very few, as we have seen). Then patient pays $40 copay while the insurer pays $10, and we providers must eat the loss. I get $50 for a treatment session at costs me $75. For my clinic business income is down 70%. I will be out of operating funds by sept and will likely close my practice. But that is just me. What about those who need the care? By ghe way, those rationing programs say the patient approved for 3-4 sessions for their back pain can then have NO therapy for that body part ever again in their lifetime.

    obamacare is here and blossoming rapidly. Next year brings lots of more new regulations to reduce ca and its costs. But at what price. You sustain another back pain episode and you cannot have any PT, and cannot pay for it out of your pocket per the rules.

    That is today's update from the front line of YOUR health care. The Soviets called it 'central planning'.

    I highly doubt Repubs will roll this back in light of the huge tax income that is part of the 2013 part of Obamacare, plusthenzeal to reduce health costs.

I just turned 60, so can begin to tap my IRA. Not enough to live on, but will turn to any sidelnes I can come up with (Registered Maine Guide, very part time teaching PT students, workplace ergonomics consulting, whatever). it will be good for me to live below poverty line for a while. I foresee underground black market health care on a private pay basis (prohibited under obamacare at some future point, by the way). F'em

    If (and a big IF) Obamacare is not repealed in its entirety, most Americans will be forced to use underground healthcare providers to receive any timely treatment in the future. What that will do for the quality of our healthcare is anybody's guess.

    Well my cane has a big sticker on it that I got from our local Rehabilitation Center that helped me learn to walk after my broken back that I am still recovering from it......Pain almost all the time....and I sport a MADE IN CHINA Sticker on my cane that I paid for with my medicare and BC/BS.

    For you central planning types we know its not about money its about control. You control the healthcare, then you can use access to coerce the people to fall into line with your destruction of the American system. You know people would sacrifice their own health but some gubmint thug type comes into your work and explains how you have to vote a certain way and your child can see the specialist they are waiting to see for their asthma.

    We know how it worked in the Soviet Union all the party types got all the healthcare and the people got whatever was left over or what they earned by falling in line.

    We have at least some recourse with Private insurance company's but with the goobmint, if they decide your parents are no longer valuable TAX payers to the system and should be made comfortable and stop receiving valuable healthcare, YOU HAVE NOBODY ELSE YOU CAN TURN TOO.

    I am still looking in the US Constitution for the Good and Plenty clause that promises the glubermint is supposed to take care of us for our own good and can't find it. Of course with NY's attack on women and their "choice" of feeding we see what the world of fascist control looks like.

    If this is anything like the experience of the HMO's of the 80's and 90's it's going to be a nightmare. I remember fighting for patients for authorization for really needed care/sevices/tests/treatments and trying to reason with a non-medical person on the other end of the line who didn't have a clue and was only interested in the blessed dollar. And that was only after waiting 72 hrs from the time the request was faxed!

    Those times were crazy making. And people died sometimes, although the hospital I was employed by during those years mostly just did what they needed to do and ate it on the costs. But unfortunately I don't see that happening this time around. Scary stuff. Really scary stuff.

    In those days, the medical groups who went with the HMO contracts and took on the medical care of whole groups of folks did get a big bonus at the end of the year if they under spent on medical costs....at least they did in my AO. That's when quality of care started suffering.

    Fortunately all the HMO's pulled out of our area...they couldn't afford to offer them here with all the senior population, at least not once there were a few wrongful deaths and lawsuits and they were held accountable and had to use medical people to authorize reasonable medical care in a reasonable amount of time. It actually took us 10 years to get the pendulum to swing back in the other direction and have any semblance of reasonable medical care for HMO patients. It was a nightmare, a pure nightmare...and I fear that it was nothing compared to what is coming.

    I'm so sorry this is happening to you Colonel Holman....you deserve better. All of us who have dedicated our lives to the healing arts deserve better.

    I am a Master's prepared Geriatric Care Manager (Bachelors in Nursing and Masters in Gerontology). I serve community dwelling older adults. In my private practice the majority of my revenue comes from private pay, with a small share of my revenue coming from contracts with long term care insurance companies to provide assessments and care planning. The word is that under Obamacare Geriatric Care Managers will be covered by Medicare and private insurance. I expect my practice will probably soar in this new venue, at least that's what I am hearing is going to happen...only because I am seen as cost effective. It's a lot cheaper to keep an elder in the community and plug in services than to place them in a facility.

    But good Lord, I do not want to even practice in that environment with lousy remibursment rates and the restrictions that will be imposed by Medicare and private insurance. Nope, not going to go there after 40 years of quality practice and care.

    Agreed, time to retire...and do some free lance work here and there. In this economy, nobody is going to have any money left for private pay very soon. And besides, private pay will probably be restricted anyway once Medicare kicks in.

    Lousy way to end a career, huh?

    We could save hundreds of millions just by STOPPING the non-emergent use of our emergency departments and implementing new national protocol standards for preventing abuse....ie 'sniffles' & mild sprains etc. AND, lets stop bleeding our taxpayers with noncitizens hammering our medical welfare systems too while were at it.

    That is where we should start saving money......NOT trying to take it from bonafide patients with actual problems that need medical attention r-f-n.

    Looks like if any of you folks need specialized care or an operation, you'd better GET IT NOW!--before all this fully kicks in.

    I'm scheduled for my 2nd corneal transplant in December---so at least I will have 2 good eyes to see with.

    I hope they will at least approve my continuing to get the anti-rejection eyedrops I will need afterward, to keep the tissue from being rejected.

    A $60 / month cost to prevent having to re-do a $20 K operation.

    Think the bean-counting idiots will be able to figure THAT one out?

    I have pretty much concluded that advanced medical care is just a temporary anomaly like retirement.

    The United States has the most expensive health care system in human history and it continues to price itself out of existence. Obamacare will just hasten the death of the system that was already dying.

    What good does it do to have a life saving procedure and rehab only to be left bankrupt afterwards which is what would happen if we didn't have subsidized healthcare already to feed the beast.

    The health care system will collapse or the rest of us will collapse trying to pay for it. No other outcomes are possible.

    Countrymouse- have you looked at all.day.chemist dot com (remove the dots... I hate even mentioning the site on the open net- I KNOW they're going to shut them down sooner or later!) for your eye drops? IF they carry them- I have no idea, although I know they do carry quite a few different eye drops- they probably are $5 or under. And everything we've ever used from them has worked perfectly.

    As far as the rest... it sucks, but I agree with Bigfootscousin- the BILLIONS of dollars wasted by Medicaid and uninsured patients taking an ambulance to the ER for an ingrown toenail, sore throat or mild sprain are a HUGE part of the problem. Granted, not everyone is comfortable doing the level of medical care that I am, but anyone who is capable of taking care of themselves at home can learn how to do basic first aid, how to care for a bad bruise or sprain, etc. But during the year I had MRSA (and spent way too much time in the ER and hospital) I observed that the demographic who BFC is discussing seem to use the ER as entertainment! It's nuts... but I heard multiple conversations along the line of "well, yeah, I've had this headache all day, but there wasn't anything good on TV (or "my boyfriend stood me up", or "I was bored")... so I decided to come to the ER and get it checked out".

    No mention of even basic home treatment like aspirin.

    And then there is the opposite extreme (probably pretty rare these days except maybe in the very isolated poor populations in the Appalachians and similar environment) like my MIL ignoring my teenage SIL's broken ankle for a week "it's only sprained". The fact that it was jet black halfway to the knee, swollen to the size of a cantaloupe and too painful to bear ANY weight at all- well, dammit, the ER would probably charge $15 for x-rays! (they HAD the money, too). I guess she was fortunate it did hurt too much to walk on it... at least she didn't damage it further. But our biggest problem these days is ignorance and it is amazingly costly. (and it's amazingly common even among fairly well educated people, too!)

    And yes, people are going to die a lot quicker. I hate to say it, but in a LOT of cases, that may be a blessing. We've got way too many folks in their 80's and 90's in complete vegetative states in hospitals and nursing homes... they are only alive because of IV's and feeding tubes. They have no awareness of ANYthing around them, and never will again. NO, I'm not suggesting euthanasia. But just because we have the ability to keep a body alive almost indefinitely once the brain has completely shut down doesn't mean we should, either...

    It's a freakin disaster, all the way around.

    The whole focus will be on treating WELL PEOPLE, supposedly to keep them well. Preventive medicine will be all they will want to spend money upon. Sick people, especially VERY sick/injured people will be given the short shrift.

    Actual high quality medical care and treatment FOR SICK AND INJURED PEOPLE will disappear, eventually for the wealthy as well as initially for the average citizen. All that will be measured out is broken bone, easy fix illness/injury treatment.
    The same Cross at which I find forgiveness for MY sins I must ALSO look to for JUSTICE for crimes committed AGAINST ME and also against other innocent people. It is where you look to and find PEACE about all the evil and injustice in this world.

    ^^^This. Very much this.

    Another angle---let's say the system collapses of its own weight. A lot of people would die because of a variety of causes (talking purely health care-related issues). OTOH, let's say the controllers get theitr uber-planned medical system, and they kill off a lot of people because those people are too expensive to treat or too old/disabled/politically incorrect. Now, some would say there is no difference because a lot of the same people end up dead either way. To me (getting up there), there's a HUGE difference in my dying of natural causes and my being murdered because it suits TPTB.
